不同模式重复经颅磁刺激对精神分裂症患者前瞻性记忆的影响

Effect of Different Modes of Repetitive Transcranial Magnetic Stimulation on Prospective Memory in Schizophrenia Patients

摘要 目的探讨不同模式重复经颅磁刺激(r TMS)对精神分裂症患者前瞻性记忆(PM)的影响。方法选取2019年1月至2020年12月在医院就诊的120例精神分裂症患者,随机分为伪刺激组、1 Hz组、10 Hz组、θ短阵快速脉冲刺激(TBS)组,各30例,分别接受伪刺激、左侧背外侧前额叶(L-DLPFC)1 Hz低频刺激、L-DLPFC 10 Hz高频刺激、TBS 4种模式r TMS治疗,比较4组治疗前及治疗4周后基于事件的前瞻性记忆(EBPM)评分和基于时间的前瞻性记忆(TBPM)评分,以及1 Hz组、10 Hz组及TBS组治疗前与治疗4周后的EBPM评分、TBPM评分差值。结果伪刺激组治疗前与治疗4周后的EBPM评分、TBPM评分比较,差异均无统计学意义(P>0.05);1 Hz组、10 Hz组及TBS组治疗前与治疗4周后的EBPM评分、TBPM评分比较,差异均有统计学意义(P<0.05);1 Hz组、10 Hz组及TBS组治疗4周后的EBPM评分、TBPM评分组间比较及两两比较,差异均有统计学意义(P<0.05);1 Hz组、10 Hz组及TBS组治疗前与治疗4周后的EBPM评分、TBPM评分差值比较,差异均有统计学意义(P<0.05),且TBS组EBPM评分、TBPM评分差值最大。结论r TMS治疗可改善精神分裂症患者的PM,且TBS治疗模式较其他模式疗效明显,有扩大临床应用的意义。 Objective The effect of different modes of repetitive transcranial magnetic stimulation(rTMS)on prospective memory(PM)in schizophrenia patients was explored.Methods 120 schizophrenic patients admitted to the hospital from January 2019 to December 2020 were selected and randomly divided into 4 groups,that is,the pseudo stimulation group,the 1 Hz group,the 10 Hz group and the theta burst stimulation(TBS)group,with 30 cases in each group.The four groups were treated by four kinds of rTMS accordingly.The pseudo simulation group received pseudo stimulation,the 1 Hz group received left dorsolateral prefrontal cortex(L-DLPFC)low frequency stimulation(1 Hz),the 10 Hz group received L-DLPFC high frequency stimulation(10 Hz)and the TBS group received TBS.Before and four weeks after treatment,the scores of the event-based PM(EBPM)and time-based PM(TBPM)among the four groups were compared.And also the differences of EBPM and TBPM scores among the 1 Hz group,the 10 Hz group and the TBS group were compared before and four weeks after treatment.Results There was no significant difference in EBPM and TBPM scores before and four weeks after treatment in the pseudo stimulation group(P>0.05);There were significant differences in EBPM and TBPM scores before and four weeks after treatment among the 1 Hz group,the 10 Hz group and the TBS group(P<0.05);There were significant differences in EBPM and TBPM scores among the three groups four weeks after treatment(P<0.05),and the differences were all statistically significant when any two groups were compared(P<0.05);There were significant differences in differences of EBPM and TBPM scores before and four weeks after treatment among the 1 Hz group,the 10 Hz group and the TBS group(P<0.05)and they were the highest in the TBS group.Conclusion r TMS can improve the PM of schizophrenic patients,and TBS mode is more effective than the other stimulation modes and it is of significance to expand its clinical application.
