摘要 目的 探讨肺表面活性物质(PS)结合允许性高碳酸血症(PHC)通气法在足月新生儿急性呼吸窘迫综合征(ARDS)中的应用效果。方法 选取2020年4月至2021年4月我院收治的70例ARDS足月新生儿,随机分为观察组(PS结合PHC通气法)与对照组(PS)。对比两组的治疗效果和并发症情况。结果 观察组的总有效率高于对照组(P<0.05)。观察组机械通气时间、氧疗时间、住院时间短于对照组(P<0.05)。观察组治疗24 h后的PaCO_(2)水平高于对照组(P<0.05)。观察组呼吸机相关性肺损伤发生率低于对照组(P<0.05)。结论 PS结合PHC通气法治疗足月新生儿ARDS的疗效较好,可缩短机械通气时间,促进患儿恢复,减少呼吸机相关性肺损伤的发生。 Objective To explore the application effect of pulmonary surfactant(PS)combined with permissive hypercapnia(PHC)ventilation in full-term neonates with acute respiratory distress syndrome(ARDS).Methods 70 full-term ARDS neonates admitted to our hospital from April 2020 to April 2021 were selected and randomly divided into observation group(PS combined with PHC ventilation)and control group(PS).The treatment effect and complications were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).The mechanical ventilation time,oxygen therapy time and hospitalization time of the observation group were shorter than those of the control group(P<0.05).The PaCO_(2) level at 24 hours after treatment of the observation group was higher than that of the control group(P<0.05).The incidence of ventilator-associated lung injury in the observation group was lower than that in the control group(P<0.05).Conclusions PS combined with PHC ventilation has better efficacy in the treatment of full-term ARDS neonates,which can shorten the mechanical ventilation time,promote the recovery of children,and reduce the ventilator-associated lung injury.
