白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障患者的效果

Effects of phacoemulsification combined with intraocular lens implantation in treatment of patients with angle-closure glaucoma and cataract

目的:观察白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障患者的效果。方法:选取96例闭角型青光眼合并白内障患者作为研究对象,按随机数字表法分为对照组48例(51只眼)和观察组48例(53只眼)。对照组采用周边虹膜切除术治疗,观察组采用白内障超声乳化联合人工晶状体植入术治疗,比较两组手术前后最佳矫正视力、眼压、前房深度、房角开放度和并发症发生率。结果:观察组最佳矫正视力水平高于对照组,眼压水平低于对照组,差异有统计学意义(P<0.05);观察组前房深度、房角开放度均高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为4.17%,明显低于对照组的18.75%,差异有统计学意义(P<0.05)。结论:白内障超声乳化联合人工晶状体植入术治疗闭角型青光眼合并白内障患者可提高最佳矫正视力水平、前房深度和房角开放度,以及降低眼压水平和并发症发生率,其效果优于周边虹膜切除术治疗。

Objective:To observe effects of phacoemulsification combined with intraocular lens implantation in treatment of patients with angle-closure glaucoma and cataract.Methods:96 patients with angle-closure glaucoma and cataract were selected as the research objects,and were divided into control group(48 cases,51 eyes)and observation group(48 cases,53 eyes)according to the random number table.The control group was treated with peripheral irisectomy,while the observation group was treated with phacoemulsification combined with intraocular lens implantation.The best corrected visual acuity,the intraocular pressure,the anterior chamber depth,the chamber open angle and the complication rate were compared between the two groups before and after surgery.Results:The best corrected visual acuity level of the observation group was higher than that of the control group;the intraocular pressure level was lower than that of the control group;and the differences were statistically significant(P<0.05).The anterior chamber depth and the chamber open angle of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 4.17%,which was significantly lower than 18.75%in the control group,and the difference was statistically significant(P<0.05).Conclusions:Phacoemulsification combined with intraocular lens implantation in the treatment of angle-closure glaucoma combined with cataract can improve the best corrected visual acuity,the anterior chamber depth and the chamber open angle,and reduce the intraocular pressure and the incidence of complications.Moreover,it is superior to peripheral iridectomy.
