摘要
目的研究二维与三维超声在婴儿髋关节发育不良(DDH)诊疗中的应用价值。方法选取2019年3月至2020年4月上饶市立医院收治的30例临床怀疑为DDH的患儿,均接受髋关节二维与三维超声检查。比较二维与三维的检查时间、患儿检查依从性及二维、三维超声检查一致性。结果两组患儿进行三维超声的检查时间短于二维超声,差异有统计学意义(P<0.05);两组患儿进行三维超声的检查依从性高于二维超声,差异有统计学意义(P<0.05);二维超声诊断敏感度为68.42%、特异性为90.91%、准确率为76.67%、阳性预测值为92.86%、阴性预测值为62.50%、Kappa值为0.541;三维超声诊断敏感度为84.21%、特异性为90.91%、准确率为86.67%、阳性预测值为94.11%、阴性预测值为76.92%、Kappa值为0.724。结论三维超声可缩短患儿检查时间,更精确观察髋关节情况,且诊断价值较高,可作为常规检查方法。
Objective To study the application value of two-dimensional ultrasound and three-dimensional ultrasound in the diagnosis and treatment of development dysplasia of hip(DDH)in infants.Methods A total of 30 children with clinically suspected DDH were selected between March 2019 and April 2020 in Shangrao Municipal Hospital.All children received two-dimensional and three-dimensional ultrasound hip examinations.The examination time of two-dimensional ultrasound and three-dimensional ultrasound,examination compliance of children,and consistency of two-dimensional ultrasound and three-dimensional ultrasound were compared.Results The examination time of the two groups of children with three-dimensional ultrasound were shorter than those of two-dimensional ultrasound,and the differences were statistically significant(P<0.05).The compliance of the two groups of children with three-dimensional ultrasound were higher than those of two-dimensional ultrasound,and the differences were statistically significant(P<0.05).Two-dimensional ultrasound diagnosis sensitivity was 68.42%,specificity was 90.91%,accuracy rate was 76.67%,positive predictive value was 92.86%,negative predictive value was 62.50%,Kappa value was 0.541.Three-dimensional ultrasound diagnosis sensitive degree was 84.21%,specificity was 90.91%,accuracy rate was 86.67%,positive predictive value was 94.11%,negative predictive value was 76.92%,Kappa value was 0.724.Conclusion Three-dimensional ultrasound can shorten the examination time and observe the condition of hip joint more accurately.It can be used as a routine examination method.
