颈2背根神经节脉冲射频治疗颈源性头痛的近期疗效 被引量:4

Efficacy of pulsed radiofrequency at dorsal root ganglion of cervical 2 for patients with cervicogenic headache

摘要 目的探讨颈2(C_(2))背根神经节脉冲射频治疗颈源性头痛患者近期疗效。方法收集2018年9月至2019年6月昆明医科大学第二附属医院疼痛科确诊为颈源性头痛患者38例,其中男17例,女21例,年龄23~65岁,进行C_(2)背根神经节脉冲射频治疗。比较治疗前和治疗结束后24 h、1周、1及3个月时视觉模拟评分(VAS)、疗效以及并发症发生情况。结果患者治疗后头痛程度VAS评分明显下降,治疗结束后24 h,1周、1及3个月时的VAS评分分别为[(3.6±0.9)、(2.4±0.6)、(2.1±0.3)、(1.8±0.7)]分,与治疗前(7.5±1.3)分比较降低明显,差异均有统计学意义(P均<0.05)。治疗前与治疗后3个月时VAS评分0~3分例数分别为0例和38例,治疗后3个月时总有效率达100%。全部患者中出现头晕2例,操作区血肿1例。结论C_(2)背根神经节脉冲射频治疗颈源性头痛具有较好的近期临床疗效。 Objective To investigate the short-term efficacy of pulsed radiofrequency at cervical 2(C_(2))dorsal root ganglion for the patients with cervicogenic headache.Methods From September 2018 to June 2019,38 patients with cervicogenic headache,17 male and 21 female,aged 23 to 65 years old,in the Department of Painology,the Second Affiliated Hospital of Kunming Medical University were treated with pulsed radiofrequency at C_(2) dorsal root ganglion.VAS,curative effect,and complications were compared before the treatment and at 24 hours,1 week,1 month and 3 months after the treatment.Results The VAS decreased significantly at 24 hours,1 week,1 month,and 3 months[(3.6±0.9),(2.4±0.6),(2.1±0.3),and(1.8±0.7)]scores respectively after the treatment than that before the treatment(7.5±1.3)(all P<0.05).The VAS was below 3 in all 38 patients at 3 months after the treatment.Two patients had dizziness and 1 patient had hematoma during the treatment.Conclusion Pulsed radiofrequency at dorsal root ganglion of C_(2) is an effective method for the treatment of patients with cervicogenic headache.
