内热针联合几丁糖关节腔注射治疗膝关节骨性关节炎的疗效

Therapeutic effect of internal heat needle combined with injection of chitosan into the knee cavity for the treatment of patients with knee osteoarthritis

目的观察膝周内热针联合膝关节腔内注射医用几丁糖治疗膝关节骨性关节炎的临床效果。方法收集苏北人民医院疼痛科2018年1月至2019年3月收治的膝关节骨性关节炎患者100例为研究对象,随机分为内热针联合几丁糖注射组(联合组,n=50)和单纯几丁糖关节腔内注射组(几丁糖组,n=50),比较治疗前与治疗后不同时点两组患者视觉模拟评分(VAS)、Lequesne指数和临床疗效。结果两组患者治疗前VAS评分和Lequesne指数差异无统计学意义(P均>0.05),两组患者治疗后VAS评分和Lequesne指数均低于治疗前,差异具有统计学意义(P均<0.05);治疗后1、4、8、24周时联合组患者的VAS评分和Lequesne指数均低于几丁糖组,差异具有统计学意义(P均<0.05);联合组治疗总有效率高于几丁糖组(92%比84%),差异具有统计学意义(P<0.05)。结论膝周内热针联合几丁糖和单纯几丁糖关节腔内注射治疗均对膝关节骨性关节炎有良好的治疗效果,但膝周内热针联合几丁糖治疗膝关节骨性关节炎疗效更好,可更有效减轻患者疼痛程度,提高患者生活质量。

Objective To observe the clinical effect of internal heat needle combined with intra-articular injection of chitosan in the treatment of knee osteoarthritis.Methods One hundred patients with knee osteoarthritis admitted to Department of Pain Medicine,Subei People's Hospital,from January 2018 to March 2019,were collected and randomly divided into two groups,internal heat needle combined with chitosan injection group(combination group,n=50)and chitosan intra-articular injection group(chitosan group,n=50).Visual analogue scale(VAS),Lequesne index score and clinical efficacy of the two groups were compared before and after treatment.Results There was no significant difference in VAS and Lequesne index score between the two groups before treatment.VAS and Lequesne index of the two groups were lower than that before treatment,and the differences were statistically significant(all P<0.05).The VAS and Lequesne index score were lower at 1 week,4 weeks,8 weeks and 24 weeks after treatment in combination group than that in chitosan group with statistical significance(all P<0.05),and the total effective rate was higher in combination group than that in chitosan group(92%vs.84%,P<0.05).Conclusion Both internal heat needle combined with chitosan injected into the knee cavity and intra-articular injection of chitosan are of good effects in the treatment of knee osteoarthritis,but the effect of internal heat needle combined with chitosan injected into the knee cavity is better than only chitosan injected into the knee cavity,with more reduction of pain and improvement of quality of life.
