摘要
目的检测微小RNA-23b(miR-23b)在变应性鼻炎患者血清中的表达水平,并探讨其表达水平与辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的关系。方法选取2017年5月—2019年3月该院耳鼻喉科确诊的变应性鼻炎患者65例为变应性鼻炎组,同期选取该院体检的健康人群60例作为对照组。采用实时荧光定量PCR(qRT-PCR)检测各组血清miR-23b表达水平;采用流式细胞术检测血清Th17和Treg细胞;采用Pearson法分析变应性鼻炎患者血清miR-23b与Th17/Treg平衡的相关性;并通过受试者工作特征曲线(ROC)评估血清miR-23b表达水平及Th17/Treg值对变应性鼻炎的诊断价值。结果变应性鼻炎组血清miR-23b表达水平显著低于对照组(P<0.05),Th17/Treg值显著高于对照组(P<0.05);变应性鼻炎患者血清miR-23b与Th17/Treg平衡呈明显负相关(r=-0.526,P<0.05);ROC结果显示,血清miR-23b、Th17/Treg值及联合诊断变应性鼻炎的曲线下面积(AUC)分别为0.905、0.860、0.964,其中联合诊断的灵敏度和特异度分别为89.2%、90.0%。结论 miR-23b在变应性鼻炎患者血清中表达水平明显降低,与Th17/Treg细胞失衡及变应性鼻炎的发生及发展密切相关,miR-23b、Th17/Treg值均可作为诊断变应性鼻炎的潜在生物学指标。
Objective To detect the expression level of microRNA-23 b(miR-23 b) in the serum of patients with allergic rhinitis, and to explore the relationship between its expression level and the balance of T helper 17 cell(Th17)/T regulatory cell(Treg).Methods From May 2017 to March 2019,sixty five patients with allergic rhinitis diagnosed in the Otolaryngology Department of Mianyang Central Hospital were selected as the allergic rhinitis group, and 60 healthy people were selected as the control group.The expression of miR-23 b was detected by real-time fluorescent quantitative PCR(qRT-PCR);the Th17 and Treg cells were detected by flow cytometry;Pearson method was used to analyze the correlation between the balance of serum miR-23 b and Th17/Treg in patients with allergic rhinitis;and the diagnostic value of serum miR-23 b expression and Th17/Treg value in allergic rhinitis was evaluated by receiver operating characteristic curve(ROC).Results The expression level of miR-23 b in the allergic rhinitis group was significantly lower than that in the control group(P<0.05),and the Th17/Treg value was significantly higher than that in the control group(P<0.05);there was a significant negative correlation between serum miR-23 b and Th17/Treg balance in patients with allergic rhinitis(r=-0.526,P<0.05);ROC results showed that the serum miR-23 b, Th17/Treg values and the area under the curve(AUC) of combined diagnosis of allergic rhinitis were 0.905,0.860 and 0.964,respectively;the sensitivity and specificity of combined diagnosis were 89.2% and 90.0%,respectively.Conclusion The expression level of miR-23 b in the serum of patients with allergic rhinitis is significantly decreased, which is closely related to the imbalance of Th17/Treg cells and the occurrence and development of allergic rhinitis.The values of miR-23 b and Th17/Treg can be used as potential biological indicators for the diagnosis of allergic rhinitis.
