3D-slicer多模态辅助下神经内镜治疗高血压脑出血及术后综合治疗的效果研究 被引量:11

STUDY ON 3D-SLICER MULTIMODALITY ASSISTED NEUROENDOSCOPY AND POSTOPERATIVE COMPREHENSIVE TREATMENT IN TREATMENT OF HYPERTENSIVE CEREBRAL HEMORRHAGE

摘要 目的观察3D-slicer多模态辅助下神经内镜治疗高血压脑出血的临床效果。方法选择2019年6月—2020年6月该院收治的高血压脑出血患者60例,并均进行了神经内镜下的血肿清除手术及术后的康复治疗,根据是否采用3D-slicer多模态辅助随机分为二组(n=30),对照组采用单纯内镜下血肿清除术,观察组则采用3D-slicer多模态辅助下神经内镜治疗高血压脑出血。并对二组治疗前、治疗后1个月、3个月分别进行神经功能缺损评分(NIHSS)、Rankin修订量表评分(mRS)及日常生活活动能力量表评分(Barthel指数)以评估治疗效果。结果术前二组NIHSS评分、mRS评分及Barthel指数差异均无统计学意义(均P>0.05),术后1个月观察组NIHSS评分、mRS评分及Barthel指数均优于对照组(P<0.05),术后3个月效果更加显著(P<0.05)。结论 3D-slicer多模态辅助下神经内镜治疗高血压脑出血不仅微创也在最大程度保住了控制肢体活动的传导束,术后早期采用综合康复治疗措施对促进患者肢体康复、改善生活能力效果显著,能有效降低致残率,能使更多的患者回归社会。 Objective To observe the clinical effect of 3 D-slicer multimodal assisted neuroendoscopy in the treatment of hypertensive cerebral hemorrhage.Methods We selected sixty patients with hypertensive cerebral hemorrhage admitted to our department from June 2019 to June 2020,and all underwent neuroendoscopic hematoma removal surgery and postoperative rehabilitation treatment.According to whether 3 D-slicermultimodal assistance, it was divided into two groups(n=30):the control group used simple endoscopic hematoma removal, and the observation group used 3 D-slicer multimodal assisted neuroendoscopy for hypertensive cerebral hemorrhage.The neurological defect score(NIHSS),Rankin Revised Scale score(mRS) and daily life activity energy power table score(Barthel index) were performed before, 1 month and 3 months after treatment to assess treatment efficacy.Results There were no significant differences in NIHSS score, mRS score and Barthel index between the two groups(P>0.05),the NIHSS score, mRS score and Barthel index were better than control group(P<0.05),and the effect at 3 months was more significant(P<0.05).Conclusion 3 D-slicer multimodal neuroendoscopic treatment of hypertensive cerebral hemorrhage not only minimally invasive also maximize the conduction of limb activity, postoperative early comprehensive rehabilitation measures to promote limb rehabilitation of patients, improve life ability, can effectively reduce the disability rate and make more patients return to society.
