糖皮质激素口服结合鼓室注射治疗全聋型突发性耳聋临床观察

Clinical Observation of Oral Glucocorticoid Combined with Tympanum Injection in Treating Sudden Deafness of Total Deafness

目的:探讨糖皮质激素口服联合鼓室注射治疗全聋型突发性耳聋的临床效果,为临床诊疗提供依据。方法:选取江门市新会区人民医院(南方医科大学附属新会医院)2017年1月至2019年12月期间收治的100例全聋型突发性耳聋患者,采用随机数表法分观察组和对照组,各50例。对照组采用糖皮质激素口服治疗,观察组在此基础上联合糖皮质激素鼓室注射治疗。观察两组患者的治疗总有效率,治疗前后平均听阈、生活质量变化,记录治疗期间不良反应。结果:观察组患者治疗总有效率为92.00 %,高于对照组的88.00 %,但差异无统计学意义(P>0.05);治疗前两组患者的平均听阈、生活质量比较,差异无统计学意义(P>0.05);治疗后两组患者平均听阈降低,生活质量评分升高,且观察组平均听阈低于对照组,生活质量评分高于对照组,差异具有统计学意义(P<0.05);观察组患者不良反应总发生率为10.00 %,低于对照组的28.00 %,差异具有统计学意义(P<0.05)。结论:糖皮质激素口服联合鼓室注射治疗全聋型突发性耳聋与口服糖皮质激素效果相当,但可更好地改善患者听力,提高生活质量,减少不良反应。

Objective To explore the clinical effect of oral glucocorticoid combined with tympanic injection in the treatment of sudden deafness of total deafness,provide evidence for clinical diagnosis and treatment.Methods 100 patients with sudden deafness in Xinhui District People’s Hospital of Jiangmen City (Xinhui Hospital Affiliated to Southern Medical University) from January 2017 to December 2019 were selected.The random number table was used to divide the observation group and the control group,each with 50 cases.The control group was treated with glucocorticoid orally,and the observation group was treated with glucocorticoid injection in the tympanum on this basis.Observe the total effective rate of the treatment of the two groups of patients,the average hearing threshold before and after the treatment,and the changes in the quality of life,and record the adverse reactions during the treatment.Results The total effective rate of treatment in the observation group was 92.00%,which was higher than 88.00% in the control group.The differences were not statistically significant (P > 0.05);Before treatment,there was no statistically significant difference in the average hearing threshold and quality of life between the two groups of patients (P > 0.05);After treatment,the average hearing threshold of the two groups of patients decreased,and the quality of life score increased,and the average hearing threshold of the observation group were lower than that of the control group,and the quality of life score were higher than that of the control group,the differences were statistically significant (P < 0.05);the total incidence of adverse reactions in the observation group was 10.00%,which was lower than 28.00% in the control group,the difference was statistically significant (P < 0.05).Conclusion Oral glucocorticoid combined with tympanic cavity injection is equivalent to oral glucocorticoid in the treatment of sudden deafness with total deafness,it can improve the patient’s hearing,and quality of life,and reduce adverse reactions.
