84例中老年人甲状腺超声及甲状腺功能检测的临床价值

Analysis of Thyroid Ultrasound and Thyroid Function in 84 Middle-aged and Elderly Patient

目的:探究中老年人群甲状腺超声及甲状腺功能检测的临床价值。方法:回顾性分析2020年4月至2021年4月来肇庆市第一人民医院体检的84例中老年人临床资料,年龄为45 ~ 75岁,其中45 ~ 55岁25例,记为Ⅰ组;56 ~ 65岁36例,记为Ⅱ组;66 ~ 75岁23例,记为Ⅲ组。观察其甲状腺超声诊断结果及甲状腺功能指标血清游离甲状腺素(FT4)、血清游离三碘甲状腺原氨酸(FT3)及促甲状腺激素(TSH)。结果:经超声诊断后,Ⅱ组、Ⅲ组的甲状腺结节检出率均高于Ⅰ组,差异具有统计学意义(P<0.05);结节人群中,Ⅱ组、Ⅲ组的多发结节检出率均高于Ⅰ组,差异具有统计学意义(P<0.05),Ⅱ组与Ⅲ组多发结节检出率差异无统计学意义(P>0.05);Ⅰ组、Ⅱ组、Ⅲ组FT4及TSH水平比较,差异无统计学意义(P>0.05);FT3组间比较,随着年龄增长指标均降低,差异具有统计学意义(P<0.05)。结论:老年人体检时超声诊断甲状腺结节准确、快速且重复性好,可作为甲状腺相关疾病较好的诊断方式,同时需了解中老年甲状腺功能特点,减少甲状腺疾病的误诊与漏诊。

Objective To explore thyroid ultrasound and thyroid function in middle-aged and elderly people.Methods The clinical data of 84 middle-aged and elderly people who underwent physical examination in the First People’s Hospital of Zhaoqing from April 2020 to April 2021 were analyzed retrospectively.The age was from 45 to 75 years old,of which 25 cases were from 45 to 55 years old,which were recorded as group I;36 cases from 56 to 65 years old were recorded as group II;23 cases from 66 to 75 years old were recorded as group III.The results of thyroid ultrasound diagnosis and thyroid function,serum free thyroxine (FT4),serum free triiodothyronine (FT3) and thyroid-stimulating hormone (TSH) were observed.Results After ultrasound diagnosis,the detection rates of thyroid nodules in groups Ⅱ and Ⅲ were higher than those in group Ⅰ,and the difference was statistically significant (P < 0.05);the detection rate of multiple nodules in group II and group III was significantly higher than that in group I(P < 0.05),but there was no significant difference between group II and group III (P > 0.05).There was no significant difference in FT4 and TSH levels among group Ⅰ,group Ⅱ and group Ⅲ (P > 0.05);FT3 among the three groups decreased with age,the difference was statistically significant (P < 0.05).Conclusion Ultrasound diagnosis of thyroid nodules during physical examination of the elderly is accurate,rapid and reproducible.It can be used as a better diagnostic method for thyroid-related diseases.At the same time,it is necessary to understand the characteristics of thyroid function in middle-aged and elderly people to reduce misdiagnosis and missed diagnosis of thyroid diseases.
