乙癸愈风汤联合针灸治疗脑卒中恢复期临床研究

Observation on curative effect of Yigui Yufeng decoction combined with acupuncture in convalescent stroke patients

目的:观察脑卒中恢复期患者采用乙癸愈风汤结合针灸治疗疗效。方法:选取脑卒中患者80例,均处于恢复期,随机分为联合组和西医组各40例,西医组患者给予西医常规治疗,联合组患者在此基础上给予乙癸愈风汤结合针灸进行联合治疗,2周为1个疗程。比较两组患者临床疗效,观察并比较两组患者治疗前后NIHSS和ADL评分、免疫功能、炎性因子水平,观察两组患者生活质量评分和并发症例数。结果:联合组治疗有效率(92.50%)高于西医组(72.50%),差异有统计学意义(P<0.05)。治疗前,联合组和西医组患者NIHSS和ADL评分比较,无统计学差异(P>0.05);治疗后两组患者NIHSS评分均降低,ADL评分均升高,且联合组降低/升高程度较西医组明显,差异有统计学意义(P<0.05)。治疗后两组患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)水平均升高,CD8^(+)水平均降低,且联合组升高/降低程度较西医组明显(P<0.05)。联合组社会功能等各项评分明显高于西医组,差异有统计学意义(P<0.05)。治疗前两组炎性因子水平比较,无统计学差异(P>0.05);治疗后两组患者IL-6、IL-8和TNF-α水平均降低,且联合组降低程度较西医组明显(P<0.05)。联合组患者不良反应发生率(7.50%)明显低于西医组(27.50%),差异有统计学意义(P<0.05)。结论:乙癸愈风汤结合针灸治疗脑卒中恢复期临床疗效显著,可改善患者神经功能缺损和日常活动能力,提高免疫功能。

Objective:Investigate the therapeutic effect of Yigui Yufeng decoction combined with acupuncture on convalescent stroke patients.Methods:80 cases of stroke convalescence patients were selected and divided into the combined group and the western medicine group according to the random number table method,40 cases each group,the western medicine group patients given conventional western medicine treatment,the combined group of patients on this basis to give Yigui Yufeng decoction combined with acupuncture combined treatment,two weeks for a course of treatment.Clinical efficacy was compared between the two groups.NIHSS and ADL scores,immune function,and inflammatory factors were observed.Quality of life scores and adverse reactions were compared between the two groups.Results:The effective rate of the combination group(92.50%)was higher than that of the western medicine group(72.50%,P<0.05).Before treatment,there was no significant difference in NIHSS and ADL scores between the combined group and the western medicine group(P>0.05).After treatment,NIHSS score and ADL score in both groups were decreased,and the degree of decrease/increase in the combined group was significantly higher than that in the western medicine group(P<0.05).There was no significant difference in immune function between the combined group and the western medicine group before treatment(P>0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups were all increased,while the levels of CD8^(+)were all decreased.Moreover,the degree of increase/decrease in the combined group was significantly higher than that in the western medicine group,with statistically significant difference(P<0.05).The scores of social function,physical life,physical function and psychological function in the combined group were significantly higher than those in the western medicine group,and the differences were statistically significant(P<0.05).There was no significant difference in inflammatory factor level between the combined group and the western medicine group before treatment(P>0.05).After treatment,the levels of IL-6,IL-8 and TNF-αin the two groups were all decreased,and the reduction degree in the combined group was significantly higher than that in the western medicine group,with statistically significant differences(P<0.05).The incidence of adverse reactions in the combined group(7.50%)was significantly lower than that in the western medicine group(27.50%,P<0.05).Conclusion:Yigui Yufeng decoction combined with acupuncture for the treatment of stroke convalescent clinical efficacy is significant,can improve the neurological impairment and daily activity of patients,improve the quality of life of patients,improve the immune function of patients.
