个案管理模式在乳腺癌患者血脂管理中的应用效果

Application effect of case management mode in blood lipid management of patients with breast cancer

目的探讨个案管理模式在乳腺癌患者血脂管理中的应用效果。方法选取2018年2月至2020年2月在本院行乳腺癌手术的183例患者为研究对象,按患者运动配合情况分为观察组75例,对照组108例,观察组患者进行中等强度有氧运动,采用个案管理,对照组无运动干预。同时在术前、术后6、12个月抽静脉血进行检测总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(Apo-A1)、载脂蛋白B(Apo B)、脂蛋白a[LP(a)]指标,比较两组患者的血脂代谢值。结果观察组与对照组血TC值比较,差异有统计学意义(P<0.05);不同运动时相的血TC值差异有统计学意义(P<0.05),随着运动时间的增加血TC呈下降趋势,运动与时相之间存在交互作用(P<0.05)。结论乳腺癌术后通过个案管理进行长期坚持有氧运动能有效降低术后TC,从而降低广泛性动脉粥样硬化源性疾病(ASCVD)的危险因素。

Objective To explore the application effect of case management model in blood lipid management of breast cancer patients. Methods A total of 183 patients who underwent breast cancer surgery in our hospital from February2018 to February 2020 were selected as the research objects. According to the patient′s exercise cooperation, they were divided into observation group of 75 cases, control group of 108 cases, and observation group of patients with moderate intensity. Aerobic exercises were managed on a case-by-case basis, and the control group had no exercise intervention.At the same time, the total cholesterol(TC), triglycerides(TG), low-density lipoprotein cholesterol(LDL-C), high-den sity lipoprotein cholesterol(LDL-C) and high-density lipoprotein cholesterol were measured by drawing venous blood before operation, 6 months after operation, and 12 months after operation.(HDL-C), Apolipoprotein A1(Apo-A1),Apolipoprotein B(Apo B), Lipoprotein a(LP(a)) indicators. Compare the blood lipid metabolism values of the two groups of patients. Results There was a difference in blood TC value between the observation group and the control group(P<0.05);there was a difference in blood TC value in different exercise phases(P<0.05);with the increase of exercise time,the blood TC showed a downward trend. There is an interaction between them,(P<0.05). Conclusion Long-term adherence to aerobic exercise through case management after breast cancer surgery can effectively reduce postoperative TC,thereby reducing the risk factors of generalized atherosclerotic disease(ASCVD).
