西妥昔单抗联合放化疗治疗宫颈癌的临床效果

CLINICAL EFFICACY OF CETUXIMAB COMBINED WITH CHEMORADIOTHERAPY IN TREATMENT OF CERVICAL CANCER

目的探讨对无法手术治疗的宫颈癌患者给予西妥昔单抗联合放化疗的临床效果。方法选取河南圣德医院2013年1月—2015年9月收治的108例无法手术治疗的宫颈癌患者作为研究对象,依据治疗方案分为基础化疗+放疗组(A组)52例和西妥昔单抗联合放化疗组(B组)56例。对两组患者治疗前后血清肿瘤标志物水平变化、临床疗效及3年时生存比例进行比较。结果B组患者的临床疗效显著高于A组(Z=10.78,P<0.05);B组患者治疗前后血清中鳞状上皮细胞癌抗原(SCC)、糖类抗原125(CA125)、癌胚抗原(CEA)水平的差值均显著高于A组(t=-27.95~-4.40,P<0.05);A组患者痤疮样皮疹、膀胱刺激征发生率明显低于B组(P=0.04、0.02);B组患者3年生存比例显著高于A组(χ^(2)=5.14,P<0.05)。结论对无法手术治疗的宫颈癌患者在基础放化疗的同时给予西妥昔单抗联合治疗,能显著降低血清肿瘤标志物的水平,并可提高患者3年生存比例。

Objective To investigate the clinical efficacy of cetuximab combined with chemoradiotherapy in patients with inoperable cervical cancer.Methods A total of 108 patients with inoperable cervical cancer who were admitted to Henan Shengde Hospital from January 2013 to September 2015 were enrolled as subjects,and according to the treatment regimen,they were divided into basic chemotherapy+radiotherapy group(group A)with 52 patients and cetuximab+chemoradiotherapy group(group B)with 56 patients.The two groups were compared in terms of changes in the levels of serum tumor markers after treatment,cli-nical outcome,and three-year survival rate.Results Group B had a significantly better clinical outcome than group A(Z=10.78,P<0.05).Compared with group A,group B had significantly greater changes in the serum levels of SCC,CA125,and CEA(t=-27.95 to-4.40,P<0.05),significantly higher incidence rates of acne-like rash and bladder irritation(P=0.04,0.02),and a significantly higher three-year survival rate(χ^(2)=5.14,P<0.05).Conclusion For patients with inoperable cervical cancer,cetuximab combined with basic chemoradiotherapy can significantly reduce the serum levels of tumor markers and improve three-year survival rate.
