面部凹陷自体脂肪颗粒移植术后移植效果不满意的影响因素分析

Analysis of Influential Factors Associated with Unsatisfactory Effect of Autologous Fat Transplantation for Facial Depression

目的分析面部凹陷患者自体脂肪颗粒移植术后移植效果不满意的相关影响因素。方法选取2018年2月至2020年2月河南科技大学第一附属医院收治的120例行自体脂肪颗粒移植术治疗的女性面部凹陷患者作为研究对象,收集患者年龄、体重指数、文化程度、凹陷类型、婚姻状况、有无营养不良、有无焦虑、有无抑郁、效果要求、有无合并感染、有无合并出血以及硬结等相关资料,根据术后6个月随访满意情况将患者分为满意组与不满意组,多因素Logistic回归分析移植效果不满意的相关影响因素。结果120例面部凹陷患者均顺利完成自体脂肪颗粒移植术,术后不满意者33例,设为不满意组,满意者87例,设为满意组。多因素Logistic回归分析结果显示,低体重指数、营养不良、效果要求过高是面部凹陷患者自体脂肪颗粒移植术后移植效果不满意的独立影响因素(95%CI为1.123~3.325、2.712~68.200、2.578~26.083,P=0.017、0.002、0.000)。结论面部凹陷患者自体脂肪颗粒移植术后移植效果满意程度与患者自身体重指数、营养状况以及效果要求密切相关。

Objective To analyze the influential factors associated with unsatisfactory effects of autologous fat transplantation in patients with facial depression.Methods 120 patients,who underwent autologous fat transplantation for facial depression in The First Affiliated Hospital of Henan University of Science and Technology between February 2018 and February 2020 were enrolled as research subjects to collect data of age,body mass index(BMI),literacy level,type of depression,marital status,nutrition status,with anxiety and depression or not,expectation towards therapeutic effects,and whether complicated with infection,bleeding and induration or not.The patients were divided into a satisfaction group and a dissatisfaction group based on the satisfaction survey at 6⁃month follow⁃up after operation,and the factors associated with unsatisfactory therapeutic effects were analyzed using multivariate Logistic regression model.Results All the 120 patients underwent the autologous fat transplantation successfully,of which 33 cases dissatisfied with the effects were set as dissatis⁃faction group,and the other 87 cases satisfied with the effects were set as satisfaction group.According to the results of mul tivariate Logistic regression analysis,low BMI,malnutrition and higher expectation towards treatment effects were independ⁃ent influential factors associated with the unsatisfactory effects of autologous fat transplantation for facial depression(95%CI:1.123-3.325,2.712-68.200,2.578-26.083,P=0.017,0.002 and 0.000).Conclusion The degree of satisfac⁃tion of patients with facial depression towards the therapeutic effects of autologous fat transplantation is closely related to patients’own body mass index,nutritional status,and expectation towards therapeutic effects.
