摘要
目的:分析气管内插管吸痰联合机械振动排痰治疗重症肺炎的疗效。方法:选取2020年1月至12月在我院PICU治疗的70例重症肺炎患儿为研究对象,按随机数字表法分为研究组和对照组,每组各35例。研究组采取气管内插管吸痰联合机械振动排痰治疗,对照组采取传统的吸痰方法联合机械振动排痰治疗。对比两组患儿在治疗前与治疗后2 h的呼吸功能指标(氧合指数、动脉血二氧化碳分压)、治疗前与治疗后24 h的炎症指标(白细胞计数、降钙素原、C反应蛋白)以及治疗有效率。结果:两组患儿治疗前的呼吸功能指标、炎症指标对比,差异均无统计学意义(P>0.05)。治疗后,研究组患儿氧合指数高于对照组,动脉血二氧化碳分压、白细胞计数、降钙素原、C反应蛋白均低于对照组,研究组治疗有效率高于对照组,差异均有统计学意义(P<0.05)。结论:对重症肺炎患儿实施气管内插管吸痰联合机械振动排痰治疗,能有效提高排痰治疗效果,改善患儿呼吸道通气功能。
Objective:To analyze the effect of endotracheal intubation sputum aspiration combined with mechanical vibration sputum expulsion in the treatment of severe pneumonia.Methods :The subjects of this study were children with severe pneumonia treated in PICU of our hospital from January 2020 to December 2020. A total of 70 children were enrolled,and they were randomly divided into study group and control group,with 35 children in each group. The study group was treated with endotracheal intubation sputum aspiration combined with mechanical vibration sputum removal,while the control group was treated with traditional sputum aspiration combined with mechanical vibration sputum removal.Resrespiratory function indexes(oxygenation index, arterial carbon dioxide partial pressure) were compared between the two groups before and 2 h after treatment. Inflammatory indexes(white blood cell count, procalcitonin, C reactive protein)and treatment response rate were compared between the two groups before and 24 h after treatment.Results :Before treatment,there were no significant differences in respiratory function indexes and inflammatory indexes between the two groups(P>0. 05). After treatment,the oxygenation index in the study group was higher than that in the control group,and the arterial partial pressure of carbon dioxide,white blood cell count,procalcitonin and C reactive protein in the study group were lower than those in the control group. The effective rate of the study group was higher than that of the control group,with statistical significance(P<0. 05).Conclusion :Endotracheal intubation and sputum aspiration combined with mechanical vibration sputum expulsion can effectively improve the treatment effect of sputum expulsion and improve the respiratory ventilation function of children with severe pneumonia.
