ICU转出病人过渡期护理照护转移质量评价指标体系的构建

Construction of evaluation index system of nursing care transfer quality of transitional care in patients transferred out from ICU

目的:构建重症监护室(ICU)转出病人过渡期护理照护转移质量评价指标体系。方法:以结构-过程-结果三维质量管理为理论框架,通过文献分析、德尔菲专家函询初步构建ICU转出病人过渡期护理照护转移质量评价指标体系,运用层次分析法确定权重,选取324名ICU护士进行指标体系的信效度检验。结果:共进行2轮专家函询,问卷有效回收率分别为92.9%和84.6%,专家权威系数为0.824,专家函询肯德尔和谐系数(Kendall′s W)值分别为0.224和0.245(P<0.001)。构建的ICU转出病人过渡期护理照护转移质量评价指标体系包括3个一级指标、15个二级指标和47个三级指标,指标体系总体Cronbach′sα系数为0.957,内容效度为0.98,探索性因子分析以特征根>1.5为标准,共提取出3个公因子,累积方差贡献率为69.236%。结论:构建的指标体系能体现ICU转出病人过渡期护理照护转移的核心内涵及基本要点,为我国护理照护转移的质量评价提供科学依据。

Objective:To construct evaluation index system of nursing care transfer quality of transitional care in patients transferred out from intensive care unit(ICU).Methods:Taken structure-process-results three-dimensional quality management as the theoretical framework,evaluation index system of nursing care transfer quality of transitional care in patients transferred out from ICU was preliminary constructed by literature analysis and Delphi expert consultation method.Weight was determined by analytic hierarchy process.And 324 ICU nurses were selected to test reliability and validity of the index system.Results:A total of 2 rounds of expert consultation method were conducted,and the effective recovery rates of questionnaires were 92.9% and 84.6%,respectively.The expert authority coefficient was 0.824.The Kendall′s W of expert consultation were 0.224 and 0.245,respectively(P<0.001).The constructed evaluation index system of nursing care transfer quality of transitional care in patients transferred out from ICU included 3 first-level indexes,15 second-level indexes,and 47 third-level indexes.The overall Cronbach′s α coefficient of the index system was0.957,and the content validity was 0.98.3 common factors were extracted when characteristic root>1.5 in exploratory factor analysis,and the cumulative contribution rate of variance was 69.236%.Conclusions:The constructed index system can reflect core connotation and basic points of nursing care transfer of transitional care in patients transferred out from ICU.It can provide scientific basis for quality evaluation of nursing care transfer in China.
