布地奈德混悬液雾化吸入对肺炎患儿心肌酶谱及血清炎性因子的影响

Effects of the atomization inhalation of budesonide suspension on myocardial zymogram and serum inflammatory factors in children with pneumonia

摘要 目的探讨布地奈德混悬液雾化吸入对肺炎患儿心肌酶谱及血清炎性因子的影响。方法56例肺炎患儿,依照随机数字表法分为对照组和观察组,每组28例。对照组患儿在常规治疗基础上予以盐酸氨溴索颗粒治疗,观察组患儿在对照组基础上予以布地奈德混悬液雾化吸入治疗。比较两组患儿治疗效果、症状消失时间及治疗前后心肌酶谱[血清乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)和肌酸激酶(CK)]、炎性因子[血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-10(IL-10)]水平。结果观察组患儿的临床总有效率为89.29%,高于对照组的53.57%,差异具有统计学意义(P<0.05)。治疗后,两组患儿血清LDH、CK-MB和CK水平较本组治疗前均下降,且观察组下降幅度大于对照组,差异具有统计学意义(P<0.05)。治疗后,两组患儿血清TNF-α、CRP水平较本组治疗前均下降,血清IL-10水平较本组治疗前升高,且观察组患儿血清TNF-α、CRP水平下降幅度及IL-10水平升高幅度均大于对照组,差异具有统计学意义(P<0.05)。观察组患儿肺部湿啰音消失时间、咳嗽消失时间、气促消失时间及发热消退时间分别为(4.16±1.03)、(3.68±0.52)、(3.23±0.66)、(3.12±0.56)d,均短于对照组的(6.99±1.37)、(6.12±1.13)、(7.98±1.03)、(5.16±0.78)d,差异具有统计学意义(P<0.05)。结论布地奈德混悬液雾化吸入能够显著改善肺炎患儿心肌酶谱、临床症状,同时可缓解患儿机体的炎症反应,临床效果显著,值得临床推广应用。 Objective To discuss the effects of the atomization inhalation of Budesonide suspension on myocardial zymogram and serum inflammatory factors in children with pneumonia.Methods 56 children with pneumonia were randomly divided into control group and observation group,with 28 cases in each group.Children in the control group were administered with ambroxol hydrochloride granules in addition to the routine treatment,while children in the observation group were administered with the atomization inhalation of budesonide suspension on the basis of the control group.Therapeutic effect,time course for symptom disappearance and levels of myocardial zymogram[serum lactate dehydrogenase(LDH),creatine kinase isoenzyme(CK-MB)and creatine kinase(CK)]and inflammatory factors[serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-10(IL-10)]before and after the treatment were compared between the two groups.Results The overall clinical effective rate of the observation group was 89.29%,which was higher than 53.57%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of serum LDH,CK-MB and CK of each group decreased compared with those before the treatment,and the decrease in the observation group was greater than that in the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of serum TNF-αand CRP of each group decreased compared with those before the treatment,while the level of serum IL-10 of each group increased compared with that before the treatment,and the decrease in levels of serum TNF-αand CRP and the increase in level of serum IL-10 of the observation group were larger than those of the control group,and the differences were statistically significant(P<0.05).The observation group had a time course for lung wet rale disappearance of(4.16±1.03)d,a time course for cough disappearance of(3.68±0.52)d,a time course for disappearance of the shortness of breath of(5.16±0.78)d,which were shorter than(6.99±1.37)(6.12±1.13),(7.98±1.03),(5.16±0.78)d of the control group,respectively;and the differences were statistically significant(P<0.05).Conclusion The atomization inhalation of Budesonide suspension may significantly improve the myocardial zymogram and clinical symptoms of children with pneumonia,and alleviate their inflammatory reaction.The clinical effect is significant.Thus,it is worthy of promotion in clinical practices.
