血清C肽联合糖化血红蛋白检验在糖尿病诊断中的价值分析 被引量:9

摘要 目的探讨血清C肽联合糖化血红蛋白检验在糖尿病诊断中的应用价值。方法80例糖尿病患者为研究对象,经胰岛素抵抗指数计算,将40例胰岛素抵抗的糖尿病患者设为抵抗组,40例胰岛素不抵抗糖尿病患者设为不抵抗组;将35例胰岛素分泌不足的糖尿病患者设为分泌不足组,45例胰岛素分泌正常的糖尿病患者设为分泌正常组;糖化血红蛋白异常的患者44例,正常的患者36例;另选取同期40例健康体检者作为对照组。所有受检者均口服葡萄糖进行耐量试验,计算胰岛素抵抗指数,比较糖尿病患者与对照组的C肽、血糖指标(空腹血糖、餐后2 h血糖)水平。结果抵抗组血清C肽、空腹血糖、餐后2 h血糖水平均高于不抵抗组与对照组,差异有统计学意义(P<0.05)。分泌不足组血清C肽水平低于分泌正常组、高于对照组,空腹血糖、餐后2 h血糖水平均高于分泌正常组与对照组,差异有统计学意义(P<0.05)。糖化血红蛋白异常患者空腹血糖及餐后2 h血糖水平分别为(9.17±1.85)、(11.95±2.15)mmol/L,均高于糖化血红蛋白正常患者的(6.30±1.12)、(6.30±1.12)mmol/L,差异有统计学意义(P<0.05)。结论糖化血红蛋白联合血清C肽检验在糖尿病患者的临床诊断中具有重要的作用,其临床价值较高,值得推广。 Objective To discuss the value of applying a test of serum C peptide combined with glycosylated hemoglobin in the diagnosis of diabetes mellitus.Methods 80 patients with diabetes mellitus were selected as study subjects.Based on their insulin resistance index,40 diabetes mellitus patients with insulin resistance were categorized as resistance group,40 diabetes mellitus patients without insulin resistance were categorized as non-resistance group;35 diabetes mellitus patients with insufficient insulin secretion were categorized as hyposecretion group,45 diabetes mellitus patients with normal insulin secretion were categorized as normal secretion group;44 patients had abnormal glycosylated hemoglobin(HbA1c),while 36 patients had normal HbA1c;40 healthy persons in the same period were selected as the control group.All subjects received oral glucose tolerance test so as to calculate the insulin resistance index.The serum C-peptide,blood glucose indexes(fasting blood glucose and 2-h postprandial blood glucose)were compared between diabetes mellitus patients and persons in the control group.Results Serum C-peptide,fasting blood glucose and 2-h postprandial blood glucose of the resistance group were higher than those of the non-resistance group and control group,and the differences were statistically significant(P<0.05).Serum C-peptide of the hyposecretion group was lower than that of the normal secretion group,and higher than that of the control group;fasting blood glucose and 2-h postprandial blood glucose of the hyposecretion group were higher than those of the normal secretion group and the control group;and the differences were statistically significant(P<0.05).The fasting glucose and 2-h postprandial glucose in patients with abnormal glycosylated hemoglobin were(9.17±1.85)and(11.95±2.15)mmol/L,which were higher than(6.30±1.12)and(6.30±1.12)mmol/L in patients with normal glycosylated hemoglobin,and the differences were statistically significant(P<0.05).Conclusion The test of glycosylated hemoglobin combined with serum C peptide plays an important role in the clinical diagnosis of patients with diabetes mellitus.It has high clinical value and is worth promoting.
