仝小林院士从“脏腑风湿”谈中医经典的传承与发展

Discussion on the inheritance and development of TCM classics from the theory of “wind-damp in zang-fu viscera” by Academician Tong Xiaolin

《医宗金鉴·凡例》指出:"医者书不熟则理不明,理不明则识不清"。中医经典如木之根,水之源,习医之人,必以研读医经为首务,博极医源,方能明辨真要,验于临床。从《黄帝内经》到近代百年,痹病的诊疗体系逐渐完备。国医大师李济仁教授是我国著名的风湿病专家,李济仁思求经旨,演其所知,突破了狭义痹病的桎梏,提出了广义痹病的内涵,其在风湿免疫性疾病诊治中的"痹痿通论,体脏互观"思想,是对《黄帝内经》理论的继承和重大发展。仝小林院士师承于李济仁教授,在李济仁学术思想的基础上,结合《内经》中的"伏邪"理论,提出了"脏腑风湿"学说。从"痹痿通论,体脏互观"到"脏腑风湿",共同构建了当代中医"痹病"新的理论体系,为包括风湿免疫类疾病在内的诸多疑难杂病的辨治开辟了新思路,展示了中医学薪火相传的大医精神,是中医经典传承与发展的生动实践。

The general notice in Yizong Jinjian (Golden Mirror of Medicine) points out “if a doctor is not familiar with medical books,he will not fully understand the theories in them,and if he doesn’t grasp the theories,he cannot make proper diagnoses and treatments in clinic”.The classics of traditional Chinese medicine are like the roots of a tree or the source of a stream,and practitioners must study them first earnestly to have an extensive knowledge of those classic theories.Only in this way can they clearly discern the true essence of a theory and put it into clinical practices.From the creation of Huangdi Neijing to modern times,the diagnosis and treatment system of arthralgia has been gradually improved and completed.Professor Li Jiren,national master of modern Chinese medicine and a well-known rheumatism expert in China,tries to tap into the treasures of the classic theories and makes full use of them in clinic,breaking through the shackles of interpreting arthralgia in a narrow sense and offering a new insight in a broad sense.His thought of “treating arthralgia and flaccidity together,and examining the syndromes in the body and viscera simultaneously” in the diagnosis and treatment of rheumatic immune diseases is an inheritance and significant development of the theory of Huangdi Neijing.Academician Tong Xiaolin inherits the thought from Professor Li Jiren and puts forward the theory of “wind-damp in zang-fu viscera” on the basis of that and in combination with the theory of “hidden pathogens” in Huangdi Neijing.Both theories have jointly constructed a new theoretical system of bi diseases in contemporary Chinese medicine,which has opened up a new idea for the syndrome differentiation and treatment of many difficult and miscellaneous diseases,including rheumatic immune diseases,reflecting the great medical spirit passed down from generation to generation in traditional Chinese medicine and representing a vivid practice of the inheritance and development of traditional Chinese medicine classics.
