生肌止痛方坐浴联合紫草纱条引流促进小儿肛周脓肿术后康复临床研究

Clinical study on promoting the postoperative rehabilitation in children with perianal abscess by the Shengji Zhitong recipe sitz bath treatment combined with the shikonin gauze drainage

目的探讨生肌止痛方坐浴联合紫草纱条引流促进小儿肛周脓肿术后康复的临床效果。方法选择肛周脓肿患儿90例作为研究对象,采用随机数表法分为A、B、C 组,各30例。3组均给予肛周脓肿一次根治术,A组术后采用高锰酸钾坐浴联合凡士林纱条引流,B组术后采用高锰酸钾坐浴联合紫草纱条引流,C组采用生肌止痛方坐浴联合紫草纱条引流。观察3组术后1、7、14 d时创面分泌物情况、创面缩小率,记录3组创面愈合时间,统计3组术后1、14 d时血清P物质(SP)、五羟色胺(5-HT)、白细胞介素-1β(IL-1β)和肿瘤坏死因子α(TNF-α)水平。结果术后7 d和14 d,B组和C组创面分泌物评分均低于A组,且C组创面分泌物评分均低于B组(均P<0.05)。术后7 d和14 d,B组和C组创面缩小率均高于A组,C组创面缩小率均高于B组(均P<0.05)。B组和C组切口愈合时间均低于A组,C组切口愈合时间低于B组(均P<0.05)。术后14 d,3组5-HT、SP、TNF-α、IL-1β水平与术前1 d时比较均显著降低(P<0.05);术后14 d,3组5-HT、SP、TNF-α、IL-1β水平差异明显(P<0.05),B组和C组均低于A组,C组水平均低于A组和B组(均P<0.05)。结论生肌止痛方坐浴联合紫草纱条引流可促进肛周脓肿患儿创面愈合,缩短愈合时间。

Objective To investigate the clinical effect of the Shengji Zhitong recipe sitz bath treatment combined with the shikonin gauze drainage on promoting the postoperative rehabilitation in children with perianal abscess.Methods A total of 90 children with perianal abscess were selected as the research subjects,and were randomly divided into groups A,B,and C,with 30 cases in each group.After all of the three groups were given a radical surgery,group A was given the potassium permanganate sitz bath treatment combined with the vaseline gauze drainage,while group B was given the potassium permanganate sitz bath treatment combined with the shikonin gauze drainage,and group C was given the Shengji Zhitong recipe sitz bath treatment combined with the shikonin gauze drainage.The wound secretion and wound reduction rate of the three groups on the 1st,7th and 14th day after operation were observed,and the wound healing time of the three groups was recorded,and the serum levels of substance P (SP),serotonin (5-HT),interleukin-1β (IL-1β) and tumor necrosis factor α (TNF-α) in the three groups on the 1st,7th and 14th day after surgery were counted.Results The wound secretion scores of groups B and C were significantly lower than those of group A on the 7th day and 14th day after surgery,and group C was significantly lower than group B (all P<0.05).The wound reduction rates of groups B and C were both significantly higher than those of group A on the 7th day and 14th day after surgery,and group C was significantly higher than group B (all P<0.05).The wound healing time of groups B and C was significantly lower than that of group A,and group C was significantly lower than group B (all P<0.05).The levels of 5-HT,SP,TNF-α and IL-1β in the three groups on the 14th day after operation were all significantly lower than those on the 1st day before operation (P<0.05).There were statistically significant differences in the 5-HT,SP,TNF-α and IL-1β levels among the three groups 14 days after operation (P<0.05),and the levels of group B and group C were lower than those of group A,and group C was lower than group A and group B,(all P<0.05).Conclusion The Shengji Zhitong recipe sitz bath treatment combined with the shikonin gauze drainage can promote the wound healing in children with perianal abscess and shorten the healing time.
