血液相关指标对诊断强直性脊柱炎的临床价值分析

Clinical Value Analysis of Blood Related Indexes in Diagnosis of Ankylosing Spondylitis

目的:分析血液学指标人类白细胞抗原B27(HLA-B27)、红细胞沉降率(ESR)、C反应蛋白(CRP)的检测结果以及疾病活动度对辅助诊断强直性脊柱炎(AS)的临床研究价值。方法:随机选取广东省第二人民医院2015年12月至2019年1月收治的129例确诊AS患者(AS组)作回顾性分析,所有患者均进行HLA-B27、ESR、CRP检测以及疾病活动度评估;并随机选取同期在本院进行健康体检的正常人52例(健康组)作为对照。结果:AS组的HLA-B27、ESR、CRP阳性率分别为85.27%、69.77%、56.59%,显著高于健康组的3.85%、9.62%、0.00%,差异均具有统计学意义(P<0.05)。以强直性脊柱炎疾病活动评分(ASDAS)将AS患者分成低疾病活动度组(66例)、高疾病活动度组(63例),高疾病活动度组患者的总体评价、医生的总体评价、评估是否需要接受强化治疗的评分、Bath强直性脊柱炎疾病活动指数(BASDAI)评分均高于低疾病活动度组,差异均具有统计学意义(P<0.05)。结论:HLA-B27指标可以用于临床辅助诊断AS,其联合ESR、CRP对分析AS患者的疾病活动、病情发展以及预后判断具有一定的指导意义。

Objective To analyze the clinical research value of hematological indexes such as human leukocyte antigen B27(HLA-B27),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and disease activity in the auxiliary diagnosis of ankylosing spondylitis(AS).Methods 129 patients with AS(AS group)treated in the Second People's Hospital of Guangdong Province from December 2015 to January 2019 were randomly selected for retrospective analysis.All patients were tested for HLA-B27,ESR,CRP and disease activity evaluation;52 normal subjects(healthy group)who underwent physical examination in our hospital in the same period were randomly selected as the control.Results The positive rates of HLA-B27,ESR and CRP in AS group were 85.27%,69.77%and 56.59%respectively,which were significantly higher than 3.85%,9.62%and 0.00%in healthy group(P<0.05).According to the ankylosing spondylitis disease activity score(ASDAS),AS patients were divided into low disease activity group(66 cases)and high disease activity group(63 cases).The overall evaluation of patients and doctors,evaluation of whether to receive intensive treatment,and Bath ankylosing spondylitis disease activity index(BASDAI)scores in the high disease activity group were higher than those in the low disease activity group,The differences were statistically significant(P<0.05).Conclusion HLA-B27 index can be used for clinical auxiliary diagnosis of AS.Its combination with ESR and CRP has certain guiding significance for analyzing the disease activity,disease development and prognosis of AS patients.
