经直肠超声检查前列腺增生症的优势及其特异性参数的临床价值

Advantages of transrectal ultrasonography in prostate hyperplasia and clinical value of specific parameters

目的判定经直肠超声检查前列腺增生(PH)的优势并分析各项参数的临床价值。方法选取2017年8月至2020年8月在延边大学附属医院泌尿外科进行PH手术治疗的患者120例作为研究对象,所有患者术前均行经腹壁(TAS)和经直肠(TRUS)两种超声检查,术中测量前列腺实际大小并结合病理诊断,比较并分析两种检查方式的不同。结果TRUS检出率为90.83%,高于TAS的75.83%,差异有统计学意义(P<0.05)。TRUS检查上下、前后及左右各径线的结果与术中测量结果比较,差异无统计学意义(P>0.05);TAS检查上下、前后及左右各径线的结果与术中测量结果比较,差异有统计学意义(P<0.05);患者年龄与TPV、TZV、TZI存在相关性(P<0.05),其中TZI的相关性最明显(P<0.01)。结论TRUS比TAS对PH患者的检出率更高,测量结果更准确,能够提供更多的有效信息。TRUS测定的特异参数与患者年龄的增长呈正相关,其中移行带指数(TZI)的相关性最为显著。

Objective To determine the advantages of transrectal ultrasonography of PH,and analyze the clinical value of each parameter.Methods A total of 120 patients who underwent PH surgery in the Department of Urology,Affiliated Hospital of Yanbian University from August 2017 to August 2020 were selected as the research objects.All patients were given two kinds of preoperative ultrasound examinations:transabdominal ultrasonography(TAS)and transrectal ultrasonography(TRUS).The actual sizes of prostate were measured intraoperatively and combined with pathological diagnosis,and the differences between the two methods were compared and analyzed.Results The detection rate of TRUS was 90.83%,which higher than 75.83%of TAS and the difference was statistically significant(P<0.05).No statistically significant differences were observed between TRUS examination results and intraoperative measurement results in checking the upper and lower,front and rear,and left and right diameter lines(P>0.05).The results of TAS examination in checking the upper and lower,front and rear,and left and right diameter lines were statistically different from those of intraoperative measurement(P<0.05).The age of patients was correlated with TPV,TZV and TZI(P<0.05),and was most significantly correlated with TZI(P<0.01).Conclusion TRUS has a higher detection rate for PH patients than TAS,and its measurement results are more accurate,which can provide more effective information.The specific parameters measured by TRUS are positively correlated with the increase of the patient age,among which the transition zone index(TZI)has the most significant correlation.
