阿尔茨海默病患者血清炎症因子的变化及临床意义

Changes and clinical significance of serum inflammatory factors in patients with Alzheimer′s disease

目的 探讨白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)在阿尔茨海默病(AD)患者血清中的表达及意义,为AD的诊断和病情监测提供依据。方法 选取2018年1月至2019年12月在台州市第一人民医院住院的95例AD患者为研究对象,另选取同期该院40名健康体检者为对照组。采用酶联免疫吸附法(ELISA)检测IL-6、TNF-α和IL-1β的浓度。采用SPSS 22.0统计学软件进行分析,多组间比较采用F检验。IL-6、TNF-α和IL-1β对AD的诊断效能采用受试者工作特征(ROC)曲线进行评价。结果 AD组患者IL-6(117.99±23.67)ng/L、TNF-α(96.70±24.35)ng/L和IL-1β(91.08±14.66)ng/L表达水平均显著高于对照组,差异有统计学意义(P<0.001)。IL-6、TNF-α和IL-1β表达水平在重度组显著高于中度组,中度组显著高于轻度组,差异均有统计学意义(P<0.001)。IL-6、TNF-α和IL-1β诊断AD的曲线下面积(AUC)分别为0.904、0.808和0.868,均具有较高的诊断价值。结论 IL-6、TNF-α和IL-1β在AD患者中表达升高,对AD的诊断和病情监测具有一定的价值。

Objective To explore the expression and significance of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)in the serum of patients with Alzheimer′s disease(AD),and to provide evidence for the diagnosis and monitoring of AD.Methods Patients with AD hospitalized in Taizhou First People′s Hospital from January 2018 to December 2019 were selected as the research objects(n=95),and healthy people undergoing physical examination in the same period were selected as the control group(n=40).Enzyme-linked immunosorbent assay(ELISA)was used to detect the concentrations of IL-6,TNF-αand IL-1β.SPSS 22.0 software was used for statistical analysis,and F test was used for comparison among multiple groups.The diagnostic efficacy of IL-6,TNF-αand IL-1βin AD was evaluated by receiver operating characteristics(ROC)curve.Results The expression levels of IL-6(117.99±23.67)ng/L,TNF-α(96.70±24.35)ng/L and IL-1β(91.08±14.66)ng/L in the AD group were significantly higher than those in the control group,with statistically significant differences(P<0.001).The expression levels of IL-6,TNF-αand IL-1βin the severe group were significantly higher than those in the moderate group,and those in the moderate group were significantly higher than those in the mild group,with statistically significant differences(P<0.001).The area under the curve(AUC)of IL-6,TNF-αand IL-1βin the diagnosis of AD were 0.904,0.808 and 0.868,respectively,which have higher diagnostic values.Conclusion The expression levels of IL-6,TNF-αand IL-1βincreased in patients with AD,which has a certain value for the diagnosis and disease monitoring of AD.
