针灸治疗中风后失语症系统评价/Meta分析再评价

Reevaluation of Systematic Evaluation/Meta-analysis of Acupuncture Treatment for Post-stroke Aphasia

目的再评价针灸治疗中风后失语症的系统评价/Meta分析文献的方法学质量和证据质量。方法计算机检索PubMed、Embase、Cochrane Library、Web of Science、中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)、中文科技期刊数据库(维普网)、中国生物医学文献数据库(CBM)建库至2020年11月24日针灸治疗中风后失语症的系统评价,由2位研究者独立进行文献筛选及数据提取,采用AMSTAR 2量表、GRADE系统评价文献质量。结果共纳入6篇系统评价/Meta分析。AMSTAR 2评价显示,所有纳入研究均存在1条以上关键条目的缺陷,方法学质量均为低级或极低级。GRADE系统分析显示,纳入文献共包含25个结局指标,证据质量评价结果提示6个结局指标为中级,其余结局指标为低级或极低级,无结局指标被评为高级。结论针灸对中风后失语症的康复具有良好疗效。但纳入研究的方法学质量和证据质量普遍较低,仍需高质量研究进一步验证。

Objective To evaluate the methodological quality and evidence quality of systematic evaluation/meta-analyses of acupuncture for the treatment of post-stroke aphasia.Methods PubMed,Embase,Cochrane Library,Web of Science,CNKI,Wangfang Data,VIP,and China Biomedical Database(CBM)were searched to collect the systematic evaluation/meta-analyses of acupuncture for the treatment of post-stroke aphasia.The retrieval time was from establishment of the databases to 24 th of November,2020.Two researchers independently carried out literature screening and data extraction.The AMSTAR 2 list and the GRADE system were used to evaluate the quality of the literature.Results Totally 6 systematic evaluation/meta-analyses were included.According to AMSTAR 2,more than 1 critical item was flawed in all included studies;therefore the methodological quality was rated as low or very low.According to GRADE,25 outcome measures were included,of which 6 were of moderate quality and the remaining were of low or very low quality,and no outcome was evaluated as high quality.Conclusion Acupuncture is effective in the rehabilitation of post-stroke aphasia.However,considering the generally low methodological quality and evidence quality of the included systematic evaluation/meta-analyses,high-quality studies are still necessary to further validate.
