强心饮配合综合治疗对慢性充血性心力衰竭老年患者血清中Galectin-3、CyPA及Copeptin水平的影响

Effect of Qiangxin Decoction and comprehensive western medicine treatment on serum Galectin-3,CyPA and Copeptin levels in elderly patients with chronic congestive heart failure

目的探讨强心饮配合综合治疗对慢性充血性心力衰竭老年患者血清中半乳糖凝集素3(Galectin-3)、亲环素(Cy PA)及和肽素(Copeptin)水平的影响。方法前瞻性选取2018年6月至2020年10月在上海市北站医院老年医学科住院治疗的慢性充血性心力衰竭患者168例,按照随机数字表法分为对照组和治疗组,各84例。对照组患者给予综合治疗(贝那普利、双氢克脲噻、琥珀酸美托洛尔),治疗组患者在对照组的基础上给予加用中药剂型强心饮治疗,两组均连续治疗15 d。观察两组患者的临床疗效及临床症状好转时间;比较两组患者的心功能的每搏输出量(SV)、心输出量(CO)、心脏指数(CI)、左心室射血分数(EVEF),血清Galectin-3、Cy PA及Copeptin水平以及不良反应发生情况。结果治疗组患者总有效率(98.81%)明显高于对照组(88.09%),差异有统计学意义(P <0.05)。治疗组患者心悸、气短、胸闷、四肢浮肿等临床症状好转时间均低于对照组,差异均有统计学意义(P <0.05)。治疗1个月后,治疗组患者的心功能SV、CO、CI、EVEF指标水平均明显高于对照组,差异均有统计学意义(P <0.05)。治疗组患者血清Galectin-3、CyPA、Copeptin水平均明显低于对照组,差异均有统计学意义(P <0.05)。治疗组不良反应总发生率(7.14%)明显低于对照组(14.29%),差异有统计学意义(P <0.05)。结论慢性充血性心力衰竭老年患者给予强心饮配合综合治疗效果明显,能有效改善心功能,并有效降低血清Galectin-3、Cy PA、Copeptin水平,且安全有效。

Objective To investigate the effect of Qiangxin Decoction combined with comprehensive treatment on the serum levels of Galectin-3( Galectin-3),cyclophilin( CyPA) and copeptin( Copeptin) in elderly patients with chronic congestive heart failure. Methods A prospective selection of 168 patients with chronic congestive heart failure who were hospitalized in the Department of Geriatrics of Shanghai Beizhan Hospital from June 2018 to October 2020. According to the random number method,they were divided into the control group and the treatment group,with 84 cases each group. Patients in the control group were given comprehensive treatment( benazepril hydrochloride tablets,dihydrocurethidium tablets,metoprolol succinate sustained-release tablets);patients in the treatment group were given the addition of Chinese medicine Qiangxin Decoction on the basis of the control group. The clinical efficacy and improvement of clinical symptoms of the two groups of patients were observed. The stroke volume( SV),cardiac output( CO),cardiac index( CI),and left ventricular ejection fraction( EVEF) of the two groups of patients,serum Galectin-3,CyPA and Copeptin levels were compared. Results The total effective rate of patients in the treatment group( 98. 81%) was significantly higher than that of the control group( 88. 09%),and the difference was statistically significant( P < 0. 05). The improvement time of clinical symptoms such as palpitations,shortness of breath,chest tightness,and edema of limbs in the treatment group was lower than the control group,the difference were statistically significant( P < 0. 05). After treatment,the levels of SV,CO,CI,and EVEF of the patients in the treatment group were significantly higher than those in the control group,and the differences were statistically significant( P <0. 05);the serum Galectin-3,CyPA,and Copeptin levels of the patients in the treatment group were lower in the control group,the differences were statistically significant( P < 0. 05). The total incidence of adverse reactions in the treatment group( 7. 14%) was significantly lower than that in the control group( 14. 29%),and the difference was statistically significant( P < 0. 05). Conclusion In elderly patients with chronic congestive heart failure,Qiangxin Decoction combined with comprehensive treatment has obvious effects,which can effectively improve heart function and effectively reduce serum Galectin-3,CyPA,and Copeptin levels,and is safe and effective.
