光动力联合咪喹莫特治疗日光性角化病临床疗效研究

Study on the Clinical Efficacy of Photodynamic Therapy Combined with Imiquimod in the Treatment of Actinic Keratosis

目的:探讨光动力(Photodynamic therapy,PDT)联合咪喹莫特治疗日光性角化病(Actinic keratosis,AK)的临床效果。方法:选取2015年1月-2020年1月于笔者医院就诊的102例AK患者。按随机数字法分为对照组和观察组,每组51例。对照组应用咪喹莫特治疗,观察组应用PDT联合咪喹莫特治疗。比较两组临床效果、皮损面积、不良反应、随访情况及满意度情况。结果:观察组总有效率(94.12%)高于对照组(70.59%),差异有统计学意义(P<0.05)。治疗后,两组皮损面积均小于治疗前,且观察组小于对照组,差异有统计学意义(P<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗后随访3~6个月,平均随访(4.59±0.70)个月,观察组复发率1.96%,低于对照组的17.65%,差异有统计学意义(P<0.05)。观察组总满意度(84.31%)高于对照组(62.74%),差异有统计学意义(P<0.05)。结论:PDT联合咪喹莫特治疗AK具有良好的临床疗效,可改善皮损情况,减小复发风险,且不增加不良反应,患者满意度高。

Objective To investigate the clinical efficacy of photodynamic therapy(PDT)combined with imiquimod in the treatment of actinic keratosis(AK).Methods Total of 102 patients with AK who were admitted to our hospital from January 2015 to January 2020 were selected.According to the random number method,they were divided into the control group(n=51)and the observation group(n=51).The control group was treated with imiquimod,and the observation group was treated with PDT combined with imiquimod.The clinical effects,skin lesion area,incidence of adverse reactions,follow-up and satisfaction were compared in the two groups.Results The total effective rate of the observation group(94.12%)was higher than that of the control group(70.59%)(P<0.05).After treatment,the skin lesion area of the two groups was smaller than before treatment,and the observation group was smaller than the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).The patients were followed up for 3 to 6 months,with an average follow-up of(4.59±0.70)months.The recurrence rate of the observation group was 1.96%,which was lower than that of the control group 17.65%(P<0.05).The total satisfaction of the observation group(84.31%)was higher than that of the control group(62.74%)(P<0.05).Conclusion PDT combined with imiquimod in the treatment of AK has a good clinical effect,which can improve the skin lesions,reduce the risk of recurrence,and does not increase adverse reactions,and has high patient satisfaction.
