牙周组织再生术联合口腔正畸术对牙周炎患者的治疗效果研究

Study on the Treatment Effect of Periodontal Tissue Regeneration Combined with Orthodontics in Patients with Periodontitis

目的:研究牙周组织再生术联合口腔正畸术对牙周炎患者的治疗效果。方法:选择2018年1月-2019年3月笔者科室治疗的152例牙周炎患者,按照随机数字表法分为两组,对照组和观察组各76例,对照组采用牙周组织再生术治疗,观察组在对照组基础上采用口腔正畸术治疗。比较两组治疗的总有效率、牙周指标、牙槽座点角度、美观满意度及并发症情况,其中,牙周指标包括菌斑指数(PLI)、牙周袋探诊深度(PPD),牙龈指数(GI),牙槽座点角度包括SNA、ANB和SNB。结果:治疗后12个月,观察组总有效率(86.8%)高于对照组(73.7%),差异有统计学意义(P<0.05)。治疗前,两组PLI、PPD、GI比较,差异均无统计学意义(P>0.05),治疗后12个月,两组以上各指标均显著降低,且观察组降低更为显著,与对照组比较差异均有统计学意义(P<0.05)。治疗前,两组SNA、ANB和SNB比较,差异均无统计学意义(P>0.05),治疗后12个月,两组以上各指标均显著改善,并且观察组改善更为显著,与对照组比较差异均有统计学意义(P<0.05)。观察组美观满意度(85.5%)显著高于对照组(69.7%),差异有统计学意义(P<0.05)。两组均无严重并发症发生。结论:牙周组织再生术联合口腔正畸术治疗牙周炎患者效果满意,能更好地改善患者的牙周指标和牙槽座点角度,美观满意度更高,具有较高的临床治疗价值。

Objective To observe the treatment effect of periodontal tissue regeneration combined with orthodontics in patients with periodontitis.Methods 152 patients with periodontitis treated in our hospital from January 2018 to March 2019 were divided into two groups according to the random number table method,76 cases in the control group and 76 cases in the observation group,the control group was treated with periodontal tissue regeneration,the Observation group was treated with orthodontics based on the control group.The total effective rate,periodontal indicators,alveolar seat angle,aesthetic satisfaction and complications of two groups were compared,among them,periodontal indicators included plaque index(PLI),periodontal pocket probing depth(PPD),gingival index(GI),and alveolar seat point angles included SNA,ANB and SNB.Results 12 months after treatment,the total effective rate(86.8%)of observation group was higher than that of control group(73.7%),but the difference was statistically significant(P<0.05).There was no statistically significant difference in PLI,PPD,GI between two groups before treatment(P>0.05),after 12 months of treatment,the above indexes of two groups were significantly reduced,and the observation group decreased more significantly,compared with the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in SNA,ANB and SNB between two groups before treatment(P>0.05),after 12 months of treatment,all above indicators of two groups were significantly improved,and the observation group improved more significantly,compared with the control group,the difference was statistically significant(P<0.05).The aesthetic satisfaction of observation group(85.5%)was significantly higher than that of control group(69.7%),and the difference was statistically significant(P>0.05).No serious complications occurred in both groups. Conclusion Periodontal tissue regeneration combined with orthodontics in the treatment of patients with periodontitis has satisfactory results, it can better improve patient's periodontal index and alveolar seat angle, it has a higher aesthetic satisfaction, and has high clinical treatment value.
