摘要
目的评价新生儿接种10μg重组乙肝疫苗10年后长期免疫效果。方法招募完成乙肝疫苗基础免疫的儿童,于2009、2014和2019年完成三次随访,检测乙肝血清学指标,单因素分析应用χ2检验,单因素分析有意义的影响因素纳入Logistic多因素分析。结果 125名儿童三次检测乙肝表面抗原阳性分别为2009年0人、2014年1人、2019年0人;乙肝表面抗体阳性率分别为100.00%、79.20%和65.60%,抗体浓度的几何均数分别为735.85、37.34和20.71 m IU/m L。乙肝表面抗体阳性率的影响因素有接种额外剂次乙肝疫苗(OR=21.960,95%CI:2.764~174.502,P=0.003)、每日体育活动时间不少于1 h(OR=3.033,95%CI:1.230~7.482,P=0.016)。结论新生儿在接种乙肝疫苗10年后,乙肝表面抗体阳性率、抗体浓度下降;接受额外剂次、适当运动能提高乙肝疫苗基础免疫的长期效果。
Objective To observe the 10-year immune persistence of hepatitis B vaccine in children who were inoculated 10μg recombinant hepatitis B vaccine. Methods Children who were inoculated 3 doses hepatitis B vaccine were recruited and completed three follow-up visits in 2009,2014 and 2019 to test hepatitis B serological indicators and conduct questionnaire.χ2 test was applied for univariate analysis,and factors that were significant for univariate analysis were included in the Logistic multivariate analysis. Results The three test result of 125 children in 2009,2014 and 2019 showed that the number of positive hepatitis B surface antigen was 0,1 and 0,respectively;the positive rate of hepatitis B surface antibody was100. 00%,79. 20% and 65. 60%,respectively;the geometric mean of antibody concentration were 735. 85 m IU/m L,37. 34 m IU/m L and 20. 71 m IU/m L,respectively. The positive rate of hepatitis B surface in children 10 years after basic vaccination was affected by additional dose of hepatitis B vaccination( OR = 21. 960,95% CI: 2. 764-174. 502,P = 0. 003) and daily physical activity time more than 1 hour( OR = 3. 033,95% CI: 1. 230-7. 482,P = 0. 016). Conclusions After 10 years of hepatitis B vaccination for newborns,the positive rate and antibody concentration will decrease;receiving additional doses and proper exercise will improve the long-term effect of hepatitis B vaccine.
