FIRM序列在评估孕晚期胎盘穿透植入的应用价值

Firm Sequences for Evaluating Placental Penetration in Late Pregnancy

目的:探讨FIRM序列在孕妇胎盘穿透性植入的应用价值。方法:回顾性分析我院40例经手术证实的胎盘穿透性植入的MRI表现及45例经手术证实的胎盘非穿透性植入的MRI影像,所有MRI序列均包含FIRM序列,将膀胱与子宫间脂肪间隙是否消失作为是否穿透植入的标准。结果:40例穿透性植入病例中,FIESTA序列与FIRM序列影像上均表现为膀胱与子宫间脂肪间隙的模糊、消失,P值大于0.05;17例植入膀胱。45例非穿透性胎盘植入中,FIESTA序列11例误判断胎盘穿透植入,造成假阳性,而FIRM序列只有5例误诊,P值小于0.05,具有统计学意义。结论:FIRM序列对于胎盘植入处邻近脂肪的显示较FIESTA序列具有良好的优势,FIESTA序列结合FIRM序列可提高对胎盘是否穿透植入的准确率。

Objective:To explore the application value of FIRM sequence in pregnant women’s placenta penetration implantation.Methods:The MRI findings of 40 cases of surgically confirmed penetrative placental implantation and 45 cases of surgically confirmed non-penetrative placental implantation in our hospital were retrospectively analyzed. All the MRI sequences contained Firm sequences, and whether the fat space between the bladder and uterus disappeared was taken as the criterion of whether the penetrative implantation was possible.Results:In 40 cases of penetrating implantation, FIESTA sequence and FIRM sequence imaging showed that the adipose space between bladder and uterus was blurred and disappeared, with P value greater than 0.05. Seventeen patients had bladder implants. Among the 45 cases of non-penetrating placenta implantation, FIESTA sequence misjudged placenta penetration implantation in 11 cases, resulting in false positive, while FIRM sequence was misdiagnosed in only 5 cases, with P value less than 0.05, which was statistically significant.Conclusion:Firm sequence has a good advantage over FIESTA sequence in the display of fat adjacent to placenta implantation, and FIESTA sequence combined with FIRM sequence can improve the accuracy of whether placenta penetration implantation.
