肠宁汤治疗肝郁脾虚型腹泻型肠易激综合征47例

Forty-Seven Cases of Diarrhea Irritable Bowel Syndrome of Liver Depression and Spleen Deficiency with Intestine-Calming Decoction

目的:观察肠宁汤治疗肝郁脾虚型腹泻型肠易激综合征的临床疗效。方法:将94例腹泻型肠易激综合征患者按照随机数字表法分为试验组和对照组,每组各47例。对照组给予奥替溴铵片联合酪酸梭菌活菌胶囊治疗,试验组在对照组治疗的基础上加用肠宁汤治疗。比较两组患者的临床疗效及治疗前后中医证候积分、肠易激综合征症状严重程度量表(IBS symptom severity scale,IBS-SSS)、汉密尔顿焦虑量表(hamilton anxiety scale,HAMA)及汉密尔顿抑郁量表(hamilton depression scale,HAMD)评分变化情况,检测两组患者治疗前后5-羟色胺(5-hydroxy tryptamin,5-HT)、白细胞介素(interleukin,IL)-6、IL-8及肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平。结果:两组患者治疗后中医证候积分、IBS-SSS评分、HAMA评分、HAMD评分低于本组治疗前,且治疗后试验组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后5-HT、IL-6、IL-8、TNF-α水平低于本组治疗前,且治疗后试验组低于对照组,差异具有统计学意义(P<0.05)。对照组有效率为74.5%,试验组有效率为93.6%,两组患者有效率比较,差异具有统计学意义(P<0.05)。结论:肠宁汤治疗肝郁脾虚型腹泻型肠易激综合征,可改善患者的临床症状,缓解焦虑、抑郁状态,降低炎性因子水平。

Objective:To observe the clinical curative effect of Intestine-Calming Decoction on diarrhea irritable bowel syndrome of liver depression and spleen deficiency syndrome.Methods:A total of 94 patients with diarrhea irritable bowel syndrome were randomly divided into the experimental group and the control group,with 47 cases in each group.The control group was treated with Otilonium Bromide Tablets combined with Clostridium Butyricum Viable Capsules,while the experimental group was treated with Intestine-Calming Decoction on the basis of the treatment of the control group.The clinical efficacy and changes of TCM syndrome score,IBS symptom severity scale(BS-SSS),Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)before and after treatment were compared between the two groups.Before and after treatment,the levels of 5-hydroxytryptamine(5-HT),interleukin(IL)-6,IL-8 and tumor necrosis factor-α(TNF-α)were detected.Results:The TCM syndrome score,IBS-SSS score,HAMA score and HAMD score of the two groups after treatment were lower than those before treatment,and the above indexes of the experimental group were lower than those of the control group after treatment,and all the differences were statistically significant(P<0.05).The levels of 5-HT,IL-6,IL-8 and TNF-αin the two groups after treatment were lower than those of the same group before treatment,and the above indexes of the experimental group were lower than those of the control group after treatment,and all the differences were statistically significant(P<0.05).The effective rate was 74.5%in the control group and 93.6%in the experimental group.There was significant difference between the two groups(P<0.05).Conclusion:Intestine-Calming Decoction can improve the clinical symptoms of patients with diarrhea irritable bowel syndrome of liver depression and spleen deficiency,alleviate anxiety and depression,and reduce the level of inflammatory factors.
