基于真实世界研究复方斑蝥胶囊辅助治疗乙型肝炎相关原发性肝癌的临床疗效

Clinical efficacy of compound cantharis capsules in adjuvant treatment of hepatitis B related primary liver cancer based on real world

目的:基于真实世界分析复方斑蝥胶囊对乙型肝炎病毒(Hepatitis B Virus,HBV)相关原发性肝细胞癌(Hepatic Cell Carcinoma,HCC)患者五年生存的影响,并进一步了解使用复方斑蝥胶囊对乙型肝炎相关肝癌不同分期、分组结局的影响,以指导临床。方法:回顾性收集河南省中医院与河南中医药大学第一附属医院2010年1月-2015年6月首次确诊为乙型肝炎相关原发性肝癌并使用复方斑蝥胶囊辅助治疗的患者,并根据年龄及性别按照1∶4的比例匹配未使用复方斑蝥胶囊及其他中成药的患者,随访患者存活时间,采集患者的人口学特征及相关临床指标信息,采用SPSS 22.0进行分析,通过COX回归分析筛选出患者生存的保护因素,并对不同分组的患者进行生存分析。结果:复方斑蝥胶囊的使用是乙型肝炎相关肝癌生存的保护因素,HR=0.385,95%CI=[0.294,0.505]。进一步分析复方斑蝥胶囊辅助治疗HCC,对不同分组患者的结局影响,P<0.05。且用药时间越长,生存时间越长,P<0.05。结论:复方斑蝥胶囊对不同分组的乙型肝炎相关性原发性肝癌患者的生存均有保护作用,且用药时间与生存时间呈正相关。

Objective:To analyze the efficacy of compound cantharis capsules on the five-year survival of patients with HBV related primary liver cancer(HCC),and further understand the effect of compound cantharis capsules on the outcome of different stages and groups of HBV related liver cancer,so as to guide the clinical practice.Methods:The patients who were first diagnosed as hepatitis B-related primary liver cancer and treated with compound cantharis capsules were retrospectively collected.The patients who did not use compound cantharis capsules and other traditional Chinese medicines were matched according to age and gender according to the proportion of 1:4.The survival time of the patients was followed up and the demographic characteristics and relevant clinical indicators were collected Information,SPSS 22.0 was used for analysis,COX regression analysis was used to screen the protective factors of patients’survival,and survival analysis was carried out for different groups of patients.Results:COX regression results showed that the use of compound cantharis capsules was the protective factor for the survival of liver cancer related to hepatitis B,HR=0.385,95%CI=[0.294,0.505].Further analysis of the effect of compound cantharis capsules on the outcome of different groups of patients,the results were statistically significant(P<0.05).The longer the time of administration,the longer the survival time(P<0.05).Conclusions:Compound cantharis capsules has protective effect on the survival of different groups of patients with hepatitis B-related primary liver cancer,and the time of administration is positively correlated with the survival time.
