摘要
目的探讨血小板联合D-二聚体预测川崎病冠脉损伤的临床价值。方法以2016年1月~2020年6月儿科收治的47例川崎病患儿为研究对象,依据超声心动图检查结果分为试验组(合并冠脉损伤,13例)和对照组(非冠脉损伤,34例),并分别于丙种球蛋白治疗前后测定两组患儿血小板指标PLT、PCT、MPV、PDW、TSP-1与TSP-2以及血D-二聚体水平,比较观察指标差异。结果治疗前,试验组PDW(16.53±0.44)%,TSP-1(34.17±10.63)μg/L、TSP-2(10962.45±3714.61)μg/L、D-二聚体(3.84±1.61)ng/L均高于对照组,差异有统计学意义(P<0.05)。治疗后,试验组PLT(349.52±96.83)×10^(9)、PCT(0.46±0.10)%、MPV(8.96±0.54)fl、PDW(16.39±0.51)%,TPS-1(27.96±5.81)μg/L,TPS-2(8022.59±3971.54)μg/L,D-二聚体(1.12±0.64)ng/L均高于对照组,差异有统计学意义(P<0.05)。结论动态监测血小板指标与D-二聚体有助于早期预测川崎病合并冠脉损伤,为临床及时诊治提供有价值的依据。
Objective To analyzed the predictive value of platelet combined with D-dimer on coronary artery damage in Kawasaki disease.Methods The 47 children with Kawasaki disease admitted to the Department of Pediatrics of our hospital from January 2016 to June 2020 were selected as the research object.According to the results of echocardiography,they were divided into test group(combined with coronary artery injury,13 cases)and control group(non-coronary Pulse injury,34 cases),the platelet indexes PLT,PCT,MPV,PDW,TSP-1 and TSP-2 and blood D-dimer levels of the two groups of children were measured before and after gamma globulin treatment,and the differences in the observed indexes were compared.Results Before treatment,the PDW(16.53±0.44)%,TSP-1(34.17±10.63)μg/L,TSP-2(10962.45±3714.61)μg/L,D-dimer(3.84±1.61)ng/L of the test group are higher than the control group,the difference is statistically significant(P<0.05).After treatment,the PLT(349.52±96.83)×10^(9),PCT(0.46±0.10)%,MPV(8.96±0.54)fl,PDW(16.39±0.51)%,TPS-1(27.96±5.81)μg/L,TPS-2(8022.59±3971.54)μg/L,D-dimer(1.12±0.64)ng/L of the test group are higher than the control group,the difference was statistically significant(P<0.05).Conclusion Dynamic monitoring of platelet indicators and D-dimer can help early prediction of Kawasaki disease complicated with coronary artery damage,and provide valuable evidence for timely clinical diagnosis and treatment.
