虎威通痹丸治疗湿热痹阻型类风湿性关节炎的临床效果

Clinical effect of Huwei Tongbi pill in the treatment of rheumatoid arthritis with dampness-heat obstruction type

目的观察虎威通痹丸治疗湿热痹阻型类风湿性关节炎的临床效果。方法选取2017年7月至2019年7月陕西中医药大学附属医院风湿免疫血液科门诊及住院部收治的68例湿热痹阻型类风湿性关节炎患者作为研究对象,按就诊顺序将其分为对照组(33例)和治疗组(35例)。对照组给予甲氨蝶呤治疗,治疗组在对照组基础上给予虎威通痹丸治疗。比较两组的临床疗效,治疗前、后的DAS28评分、关节疼痛数目、关节肿胀数目、晨僵时间、中医证候积分、ESR、CRP、RF、CCP水平及左、右手腕关节滑膜厚度;分析两组治疗期间的不良反应发生情况。结果治疗组的治疗总有效率为91.43%,显著高于对照组的69.70%(P<0.05)。治疗后,两组的DAS28评分、关节疼痛数目、关节肿胀数目、中医证候积分均较治疗前降低,晨僵时间均较治疗前缩短,且治疗组优于对照组(P<0.05)。治疗后,两组的ESR及CRP、RF、CCP水平均较治疗前降低,且治疗组低于对照组(P<0.05)。治疗后,两组的左、右手腕关节滑膜厚度均较治疗前降低,且治疗组低于对照组(P<0.05)。治疗期间,两组的不良反应均较轻微。结论虎威通痹丸治疗湿热痹阻型类风湿性关节炎的效果较好,可缓解关节肿胀、疼痛,缩短晨僵时间,改善中医证候,降低ESR及CRP、RF、CCP水平,安全性较好。

Objective To observe the clinical effect of Huwei Tongbi pill in the treatment of rheumatoid arthritis with dampness-heat obstruction type.Methods From July 2017 to July 2019,68 patients with rheumatoid arthritis of dampness-heat obstruction type admitted in the outpatient and inpatient department of rheumatic immune hematology department of affiliated hospital of Shaanxi university of Chinese medicine were selected as the research objects.The patients were divided into control group(33 cases) and treatment group(35 cases) according to the order of treatment.The control group was treated with methotrexate,and the treatment group was treated with Huwei Tongbi pill on the basis of the control group.The clinical efficacy,DAS28 score,the number of joint pain,the number of joint swelling,morning stiffness time,TCM syndrome score,ESR,CRP,RF,CCP levels and synovial thickness of left and right wrist joints before and after treatment were compared between the two groups,and the occurrence of adverse reactions during treatment was analyzed.Results The total effective rate of treatment in the treatment group was 91.43%,which was significantly higher than 69.70% in the control group(P<0.05).After treatment,DAS28 score,the number of joint pain,the number of joint swelling and TCM syndrome score in the two groups were lower than those before treatment,the morning stiffness time was shorter than that before treatment,and those in the treatment group were better than the control group(P <0.05).After treatment,the ESR,CRP,RF and CCP levels in the two groups were lower than those before treatment,and those in the treatment group were lower than the control group(P<0.05).After treatment,the synovial thickness of left and right wrist joints in the two groups were lower than those before treatment,and those in the treatment group were lower than the control group(P <0.05).During the treatment,the adverse reactions of the two groups were mild.Conclusion Huwei Tongbi pill in the treatment of rheumatoid arthritis with dampness-heat obstruction type has better effect,it can alleviate joint swelling and pain,shorten morning stiffness time,improve TCM syndrome,reduce ESR,CRP,RF and CCP levels,and has good safety.
