张春红教授运用“三维立体经络辨证法”治疗腰背痛之经验

目的:总结张春红教授临床治疗腰背痛的经络辨证方法,探索其中医理论依据,加以继承和推广。方法:通过临床跟师学习以及对张春红教授临证案例的分析和思考,结合张教授的指导,总结其诊治腰痛的经验,挖掘其经络辨证的方法和思路,文中列举出张春红教授诊治顽固性腰痛、剧烈腰痛各1例,说明“三维立体经络辨证法”的具体应用。结果:张春红教授认为十二经脉、奇经八脉均可导致腰痛,腰痛常伴随多经脉的异常,腰痛的治疗除了重视纵行的经脉外,横行的带脉及维脉也是治疗顽固性、难治性腰痛需要兼顾的重要经脉,而且腰腹是一个整体的立体结构,且腹部的任脉、肾经、脾经、胃经等都是导致腰痛的重要经脉,腰痛的诊治应重视腹部经脉的触诊及针刺治疗,总之张教授认为治疗腰痛应从纵行、横行及腹部经脉“三维立体经络辨证法”进行定病位和针刺选穴,疗效显著。结论:其思路独特、经验丰富,“三维立体经络辨证法”是适合腰痛的针灸治疗辨证法,临证具有重要的指导意义,值得推广。

Objective:To summarize the meridian differentiation method of Professor Zhang Chunhong's clinical treatment of low back pain,to explore the theoretical basis of Chinese medicine,to inherit and promote it.Methods:Through clinical study with the teacher and analysis and thinking of Professor Zhang Chunhong's clinical cases,combined with Professor Zhang's guidance,his experience in the diagnosis and treatment of low back pain is summarized,and his meridian differentiation methods and ideas are explored.This article lists the cases of intractable lumbago and severe lumbago treated by Professor Zhang Chunhong and explains the specific application of the"three-dimensional meridian differentiation method".Result:Professor Zhang Chunhong holds that the pathological changes of the twelve meridians and the eight extra-meridians can cause low back pain.Low back pain is often accompanied by abnormalities of multiple meridians.In the treatment of low back pain,besides focusing on the longitudinal meridians,the transverse and venous meridians are also intractable and difficult to treat.The important meridians need to be considered in the treatment of low back pain,and the waist and abdomen are a whole three-dimensional structure,and the Ren,kidney,spleen,and stomach channels in the abdomen are all important meridians that cause low back pain.The diagnosis and treatment of low back pain should pay attention to palpation and acupuncture of abdominal meridians.In general,Professor Zhang believes that the treatment of low back pain should be based on the"three-dimensional meridian differentiation method"of longitudinal,transverse and abdominal meridians to locate the disease and select acupoints for acupuncture,of which the effect is significant.Conclusion:Professor Zhang Chunhong's has unique ideas and rich experience in the clinical treatment of low back pain.The"three-dimensional meridian differentiation method"is a suitable acupuncture and moxibustion syndrome differentiation method for low back pain.It has important clinical significance and is worthy of promotion.
