自拟健脾止泻汤加减联合西医对症支持治疗在小儿腹泻伴脱水中的应用

The application of Self-made modified Jianpi Zhixie decoction combined with Western medicine for symptomatic and supportive treatment in children with diarrhea and dehydration

目的:探讨对腹泻病伴脱水的小儿采取自拟健脾止泻汤加减联合西医对症治疗的临床效果。方法:小儿腹泻是儿内科常见的一种由多病原、多因素引起的疾病,常见于2岁以下的婴幼儿。自身因素(如肠道功能发育不完全、营养不良等)、消化道感染、肠道菌群及消化功能紊乱等是小儿腹泻常见的致病因素,该病的临床症状以腹泻为主,通常伴有脱水。选取济南市济阳区中医医院儿科2018年1月-2019年12月收治的204例出现腹泻伴脱水的小儿患者作为研究对象,并通过随机数字表法将其均分为观察组和对照组各102例,研究所选的研究对象均获取患者、家属的知情同意,并且也都已通过伦理委员会的筛选。其中对照组患儿采取西医对症治疗,观察组患儿采取健脾止泻汤加减联合西医对症治疗,统计两组患儿的治疗有效率。结果:观察组患儿的临床治疗有效率(99/102,97.06%)明显优于对照组患儿的临床治疗有效率(84/102,82.34%),差异具有统计学意义(χ^(2)=1.991,P<0.05)。结论:观察组在住院、脱水纠正和腹泻改善时间均优于对照组。故健脾止泻汤加减联合西医对症治疗可有效提高小儿腹泻伴脱水的临床治疗有效率,减少相关并发症的发生,并且患儿预后良好,而且中药无不良反应,不易产生药物依赖,值得在临床上进行应用和推广。

Objective:To explore the clinical effect of self-made modified Jianpi Zhixie decoction(健脾止泻汤)combined with Western medicine for symptomatic and supportive treatment in children with diarrhea and dehydration.Methods:Infantile diarrhea is a common disease in pediatrics,which is caused by many pathogens and many factors.It is common in infants under 2 years old.AUTOGENIC factors(such as intestinal dysgenesis,malnutrition,etc.),digestive tract infection,intestinal flora and digestive dysfunction are the common therapeutic factors for infantile diarrhea,the clinical symptoms of which are mainly diarrhea,which is usually accompanied by dehydration.A total of 204 pediatric patients with diarrhea and dehydration from January 2018 to December 2019 were selected and divided into two groups by random number table(102 cases),informed consent was obtained from patients and their families,and all of the subjects were screened by the ethics committee.The control group was treated with Western medicine,and the observation group was treated with self-made modified Jianpi Zhixie decoction combined with Western medicine.Results:The clinical effective rate of the observation group(99/102,97.06%)was significantly higher than that of the control group(84/102,82.34%),the difference was statistically significant(χ^(2)=1.991,P<0.05).Conclusion:The time of hospitalization,dehydration correction and diarrhea improvement in the observation group was better than that in the control group.Therefore,self-made modified Jianpi Zhixie decoction combined with Western medicine can effectively increase the effective rate of clinical treatment of infantile diarrhea with dehydration,reduce the occurrence of related complications,and children are with good prognosis,and it has no toxic side effects,which is not easy to produce drug dependence,and is worthy of clinical application and promotion.
