活血化瘀类中药注射剂治疗缺血性脑卒中的临床价值

Clinical value of promoting blood circulation and removing blood stasis TCM injections in treating patients with ischemic stroke

目的:探讨常用的活血化瘀类中药注射剂治疗缺血性脑卒中的有效性及安全性。方法:选取2017年1-2019年12月长春市中医院就诊的缺血性脑卒中患者366例作为研究对象,按照患者就诊的顺序,将研究对象随机分为六组,分别为血塞通组、灯盏花素组、疏血通组、醒脑静组、丹参川芎嗪组和丹红组,比较不同活血化瘀类中药注射剂治疗的疗效和安全性。结果:治疗前各组患者美国国立卫生院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分和日常生活能力评分(Activities of Daily Living,ADL)评分比较,差异无统计学意义(P>0.05),治疗后各组NIHSS和ADL评分较治疗前明显降低(P<0.05),其中丹红组最低,灯盏花素组最高。丹红组临床疗效最高,为78.69%(48/61),灯盏花素组最低,为55.74%(34/61),均与其他组有明显差异(P均<0.05),各组均未出现明显肝肾功能改变,各组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:活血化瘀类中药注射剂尤其是丹红注射液治疗缺血性脑卒中具有明显临床疗效,能够改善神经功能和日常生活能力,具有较高安全性。

Objective:To explore the safety and safety of commonly used TCM injections for promoting blood circulation and removing blood stasis in the treatment of ischemic stroke.Methods:366 patients with ischemic stroke who were treated in Changchun Traditional Chinese Medicine Hospital from January 2017 to December 2019 were selected as the study subjects.According to the order of the patients'treatment,the subjects were randomly divided into six groups,namely Xuesaitong(血塞通)group and Dengzhanhuasu(灯盏花素)group,Shuxuetong(疏血通)group,Xingnaojing(醒脑静)group;Danshen Chuanxiongqin(丹参川芎嗪)group and Danhong(丹红)group,the efficacy and safety of different TCM injections for promoting blood circulation and removing blood stasis were compared.Results:There was no significant difference in NIHSS and ADL scores between the two groups before treatment(P>0.05).After treatment,the NIHSS and ADL scores of each group were significantly lower than before treatment(P<0.05).Among them,the Danhong group was the lowest and the Dengzhanhuasu group was the highest.The clinical efficacy of Danhong group was the highest of 78.69%(48/61),and the Dengzhanhuasu group was the lowest of 55.74%(34/61),which were significantly different from other groups(P<0.05).There were no obvious changes in liver and kidney function in each group.There was no significant difference in the incidence rate(P>0.05).Conclusions:TCM injections for promoting blood circulation and removing blood stasis,especially Danhong injection has obvious clinical effects on nerve function improvement and it has high safety in the treatment of ischemic stroke.
