摘要 目的利用高分辨率磁共振研究颅内动脉管壁及斑块特点与急性脑梗死早期神经功能恶化(END)的关系。方法收集51例颅内动脉粥样硬化斑块所致急性脑梗死患者并进行高分辨磁共振扫描。根据脑梗死发病后72 h内患者是否发生END,分为END组和non-END组,比较两组患者颅内动脉血管管壁及斑块的差别。结果 END组患者的斑块长度较non-END组更短[6.35(9.54)vs 10.75(9.73),P<0.05],END组血管重构指数低于non-END组[(0.917 3±0.146 8)vs(1.010 9±0.125 1),P<0.05],END组负性重构的比例高于non-END组[56.52%vs 28.57%,P<0.05]。结论急性脑梗死END与血管重构及斑块长度密切相关。 Objective To study the relationship between intracranial arterial wall and plaque characteristics and early neurological deterioration(END) in acute ischemic stroke by high resolution magnetic resonance imaging(HRMRI). Methods 51 patients with acute ischemic stroke caused by intracranial atherosclerotic plaque were collected and high resolution magnetic resonance scanning was performed. The patients were divided into the END group and the non-End group according to whether END occurred within 72 hours after the onset of cerebral infarction, and the differences of intracranial artery wall and plaque characteristics between the two groups were compared. Results The plaque length in the END group was shorter than that in the non-End group [6.35(9.54) vs 10.75(9.73), P<0.05], and the vascular remodeling index in the END group was lower than that in the non-End group [(0.917 3±0.146 8) vs(1.010 9±0.125 1), P<0.05]. The proportion of negative remodeling in the END group was higher than that in the non-end group [56.52% vs 28.57%, P<0.05]. Conclusion END is closely related to vascular remodeling and plaque length in acute ischemic stroke.
