子宫内膜异位症患者异位内膜组织NFAT、FOXP3表达及其临床意义

Expression of NFAT and FOXP3 in ectopic endometriosis of patients with endometriosis and its clinical significance

目的探讨子宫内膜异位症(EMT)患者异位内膜组织中活化T细胞核因子(NFAT)、叉头框蛋白P3(FOXP3)的表达及其临床意义。方法选取2017年1月至2018年6月北京市朝阳区妇幼保健院收治的行宫腔镜联合腹腔镜手术的68例EMT患者作为EMT组,另选取同期因子宫肌瘤行子宫全切的60例患者作为对照组。免疫组化法检测内膜组织中NFAT、FOXP3的表达;EMT组患者进行术后3年随访,采用Kaplan-Meier生存曲线分析NFAT、FOXP3表达与EMT复发的关系。结果 EMT组异位内膜组织中NFAT、FOXP3阳性表达率显著高于对照组正常内膜组织(P<0.05);EMT组不同分期患者NFAT、FOXP3阳性表达率比较,差异具有统计学意义(P<0.05),且随着分期的增加,患者NFAT、FOXP3阳性表达率显著增加(P<0.05);EMT组患者NFAT和FOXP3阳性表达呈正相关(r=0.373,P<0.05);NFAT、FOXP3阳性表达患者EMT术后3年复发率高于阴性表达患者,但差异无统计学意义(P>0.05)。结论 EMT患者异位内膜组织中NFAT、FOXP3阳性表达率显著高于正常内膜组织,与患者EMT分期有关,而与患者3年内是否复发无显著相关性。

Objective To investigate the expression and clinical significance of nuclear factor of activated T cells(NFAT) and forkhead box P3(FOXP3) in ectopic endometrial tissue of patients with endometriosis(EMT). Methods 68 EMT patients who underwent hysteroscopic and laparoscopic surgery in Beijing Chaoyang District Maternal and Child Health Hospital from January 2017 to June 2018 were selected as the EMT group, and 60 patients who underwent total hysterectomy for uterine fibroids during the same period were selected as the control group. Immunohistochemistry was used to detect the expression of NFAT and FOXP3 in endometrial tissues. The EMT group was followed up 3 years after operation. Kaplan-Meier survival curve was used to analyze the relationship between NFAT and FOXP3 expression and EMT recurrence. Results The positive expression rates of NFAT and FOXP3 in ectopic endometrial tissues in EMT group were significantly higher than those in control group(P<0.05). The positive expression rates of NFAT and FOXP3 in patients with different stages of EMT group were significantly different(P<0.05), and the positive expression rates of NFAT and FOXP3 were significantly increased with the increase of stage(P<0.05). There was a positive correlation between NFAT and FOXP3 expression in EMT group(r=0.373, P<0.05). The recurrence rate of patients with positive NFAT and FOXP3 expression was higher than that of patients with negative expression, but the difference was not statistically significant(P>0.05). Conclusions The positive expression rates of NFAT and FOXP3 in ectopic endometrial tissues of EMT patients were significantly higher than that in normal endometrial tissues, which was related to EMT stage of patients, but had no significant correlation with recurrence within 3 years.
