黄芪祛疣汤联合二氧化碳激光术对尖锐湿疣患者局部人乳头瘤病毒载量、血清干扰素-γ和转化生长因子-β水平及数字分级评分的影响 被引量:8

The effects of Huangqi Quyou Decoction combined with CO;laser on local HPV load, serum IFN-γ and TGF-β levels and NRS score in patients with condyloma acuminatum

摘要 目的探讨黄芪祛疣汤联合二氧化碳(CO_(2))激光术对尖锐湿疣患者局部人乳头瘤病毒(HPV)载量、血清干扰素-γ(IFN-γ)、转化生长因子-β(TGF-β)水平及数字分级评分(NRS)的影响。方法选取2018年5月至2019年5月青海省中医院疮疡科诊治的92例尖锐湿疣患者作为研究对象。采用随机数字表法将其分为对照组和观察组,各46例。对照组接受CO_(2)激光术治疗,观察组在对照组治疗基础上接受黄芪祛疣汤治疗,两组均连续治疗2个月。比较两组疗效、复发率、治疗前后局部HPV载量、血清IFN-γ、TGF-β水平及疼痛情况,观察两组不良反应发生情况。结果观察组治疗总有效率明显高于对照组,差异具有统计学意义(P<0.05)。观察组复发率明显低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组局部HPV载量及血清TGF-β水平均明显低于治疗前,且观察组明显低于对照组,两组血清IFN-γ水平均明显高于治疗前,且观察组明显高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组NRS均明显高于治疗前,但观察组明显低于对照组,差异具有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论黄芪祛疣汤联合CO_(2)激光术治疗尖锐湿疣的疗效好,且可有效降低患者局部HPV载量,改善炎症因子水平,安全性较高。 Objective To investigate the effects of Huangqi Quyou Decoction combined with carbon dioxide(CO_(2)) laser on local human papilloma virus(HPV) load, serum interferon-γ(IFN-γ) and transforming growth factor-β(TGF-β) levels and numerical rating scale(NRS) score in patients with condyloma acuminatum. Methods 92 patients with condyloma acuminatum treated in the Department of Sores and Ulcers of Qinghai Hospital of T.C.M from May 2018 to May 2019 were selected, and divided into control group and observation group with 46 cases in each group by random number table method. The control group received CO_(2)laser treatment, and the observation group received Huangqi Quyou Decoction on the basis of treatment in the control group. Both groups were continuously treated for 2 months. Efficacy, recurrence rate, local HPV load, serum IFN-γ and TGF-β levels and pain before and after treatment were compared between the two groups, and the occurrence of adverse reactions between the two groups was observed. Results The total effective rate of observation group was significantly higher than that of control group(P<0.05). The recurrence rate of observation group was significantly lower than that of control group(P<0.05). After treatment, local HPV load and serum TGF-β levels in both groups were significantly lower than before, and observation group was significantly lower than control group, while serum IFN-γ levels in both groups were significantly higher than before, and observation group was significantly higher than control group, the differences were statistically significant(P<0.05). NRS in both groups were significantly higher than before, but the observation group was significantly lower than the control group, the difference was statistically significant(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusions Huangqi Quyou Decoction combined with CO_(2)laser in the treatment of condyloma acuminatum can improve the efficacy, effectively reduce the local HPV load of patients, improve the level of inflammatory factors, and has high safety.
