溶血标本对标本中生化指标准确性的影响分析 被引量:1

摘要 目的讨论标本溶血对生化检验结果产生的影响。方法120例进行身体检查的健康体检者,均抽取空腹静脉血4 ml,并将静脉血标本分为2份,每份2 ml,选取其中120份标本作为常规组,另120份标本作为溶血组。常规组进行常规的生化检验,溶血组诱导溶血之后进行生化检验。比较两组天门冬氨酸氨基转移酶(AST)、谷丙转氨酶(ALT)、乳酸脱氢酶(LDH)、钾离子(K^(+))、总胆固醇(CHOL)、碱性磷酸酶(ALP)、甘油三酯(TG)、尿素氮(BUN)、肌酐(Cr)、血糖(CLU)、血尿酸(UA)的差异。结果溶血组的AST、ALT、LDH、K^(+)、CHOL分别为(60.33±1.03)U/L、(68.04±2.05)U/L、(228.46±2.94)U/L、(5.66±0.97)mmol/L、(7.78±1.06)mmol/L,均高于常规组的(43.02±1.13)U/L、(55.64±1.12)U/L、(102.16±2.64)U/L、(3.26±0.33)mmol/L、(4.53±0.84)mmol/L,ALP(81.97±1.88)U/L低于常规组的(129.33±2.35)U/L,差异有统计学意义(P<0.05)。溶血组的TG、BUN、Cr、GLU、UA和常规组比较,差异无统计学意义(P>0.05)。结论血液标本出现溶血会对AST、ALT、LDH、K^(+)、CHOL、ALP指标产生明显的影响,在一定程度上会影响血液标本检验的准确性,容易对疾病误诊,因此临床要采取措施避免溶血标本出现。 Objective To discuss the effect of hemolytic specimen on the results of biochemical test.Methods 120 healthy person who came to our hospital for physical examination,all took 4 ml of fasting venous blood,and the venous blood specimens were divided into two portions of 2 ml each,of which 120 specimens were selected as the conventional group and the other 120 specimens were selected as the hemolysis group.Conventional biochemical tests were performed in the conventional group,and biochemical tests were performed in the hemolysis group after induction of hemolysis.The differences in aspartate aminotransferase(AST),glutamate aminotransferase(ALT),lactate dehydrogenase(LDH),potassium ion(K^(+)),total cholesterol(CHOL),alkaline phosphatase(ALP),triglycerides(TG),blood urea nitrogen(BUN),creatinine(Cr),blood glucose(CLU),and blood uric acid(UA)were compared between the two groups.Results The AST,ALT,LDH,K^(+),CHOL in the hemolysis group were(60.33±1.03)U/L,(68.04±2.05)U/L,(228.46±2.94)U/L,(5.66±0.97)mmol/L,(7.78±1.06)mmol/L,which were higher than(43.02±1.13)U/L,(55.64±1.12)U/L,(102.16±2.64)U/L,(3.26±0.33)mmol/L,(4.53±0.84)mmol/L,and the ALP(81.97±1.88)U/L of the hemolysis group was lower than(129.33±2.35)U/L of the conventional group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in TG,BUN,Cr,GLU,UA between the hemolysis group and the conventional group(P>0.05).Conclusion Hemolysis of blood specimen will have a significant impact on AST,ALT,LDH,K^(+),CHOL,ALP indexes in the specimen,and to a certain extent will affect the accuracy of blood specimen test,which is easy to lead to misjudgment in the diagnosis of diseases.Therefore,clinical measures should be taken to avoid hemolytic specimens.
