血清TNF-α、HMGB1与糖尿病合并带状疱疹疼痛程度的相关性初步探讨

摘要 目的探讨血清中肿瘤坏死因子-α(TNF-α)、高迁移率蛋白(HMGB1)与急性期带状疱疹及2型糖尿病合并带状疱疹疼痛的相关性。方法采用酶联免疫吸附法(ELISA)检测急性期带状疱疹患者25例,2型糖尿病合并带状疱疹20例和健康对照组30例患者血清中TNF-α、HMGB1的水平。结果急性期带状疱疹组和2型糖尿病合并带状疱疹组TNF-α浓度分别为(12.03±5.6)pg/mL、(13.62±2.23)pg/mL,高于对照组的(6.08±1.40)pg/mL,且2型糖尿病合并带状疱疹组TNF-α浓度高于急性期带状疱疹组,差异具有统计学意义(P<0.05);2型糖尿病合并带状疱疹组HMGB1浓度(16.12±2.22)ng/mL高于急性期带状疱疹组(7.80±2.24)ng/mL和健康对照组(6.87±2.15)ng/mL,差异具有统计学意义(P<0.05);急性期带状疱疹及2型糖尿病合并带状疱疹重度组TNF-α浓度分别为(13.37±1.16)、(19.84±1.39)pg/mL,高于轻中度组(9.61±2.87)、(18.18±2.08)pg/mL(P<0.05);急性期带状疱疹及2型糖尿病合并带状疱疹轻中度组与重度组间HMGB1浓度差异无统计学意义(P>0.05);急性期带状疱疹组及2型糖尿病合并带状疱疹组中疼痛VAS评分与血清TNF-α浓度有一定相关性(r=0.652及0.569,P<0.05),与HMGB1无相关性(r=0.114及0.366,P>0.05)。结论TNF-α可能在急性期带状疱疹及糖尿病合并带状疱疹患者疼痛的发生中起一定的作用;HMGB1可能与糖尿病合并带状疱疹的患者的疼痛具有相关性。 Objective To investigate the correlation between serum TNF-a, HMGB1 and herpes zoster pain in acute stage and type 2 diabetes mellitus(T2 DM).Methods Serum TNF-α and HMGB1 were detected by enzyme-linked immunosorbent assay(ELISA)in 25 patients with acute stage herpes zoster, 20 patients with type 2 diabetes mellitus with herpes zoster and 30 healthy controls.Results The concentration of TNF-α in acute stage herpes zoster group and T2 DM with herpes zoster group was(12.03±5.6)pg/mL and(13.62±2.23)pg/mL,respectively, which were higher than that of the control group(6.08±1.40)pg/mL. The concentration of TNF-α in T2 DM complicated with herpes zoster group was higher than that in acute stage herpes zoster group(P<0.05). The concentration of HMGB1 in T2 DM patients with herpes zoster(16.12±2.22)ng/mL was higher than that in acute stage group(7.80±2.24)ng/mL and healthy control group(6.87±2.15)ng/mL,and the difference was statistically significant(P<0.05).The concentration of TNF-α in acute stage herpes zoster and T2 DM with severe herpes zoster was(13.37±1.16,19.84±1.39)pg/mL,respectively, which was higher than that in mild and moderate group(9.61±2.87,18.18±2.08)pg/mL,and the difference was statistically significant(P<0.05). There was no significant difference in HMGB1 concentration between acute herpes zoster and T2 DM combined with mild and moderate herpes zoster and severe herpes zoster(P>0.05). There was a well correlation between the VAS score and serum TNF-α concentration in the acute stage and T2 DM with herpes zoster(r=0.652 and 0.569,P<0.05),but no correlation with HMGB1(r= 0.114 and 0.366,P>0.05).Conclusion TNF-α may play a role in the development of pain in patients with acute herpes zoster and diabetes mellitus complicated with herpes zoster. HMGB1 may be associated with pain in patients with diabetes and herpes zoster.
