早期活动对心外ICU患者呼吸机相关性肺炎的预防效果

Preventive effect of early activities on ventilator-associated pneumonia in ICU patients with cardiovascular surgery

目的探究早期活动对心外ICU患者呼吸机相关性肺炎的预防效果。方法选取接受成人心外科手术患者300例,采用随机数字表法将患者分为实验组和对照组,每组150例。对照组采用常规护理方法,试验组在对照组的基础上进行早期活动干预。观察指标包括各项呼吸指标、呼吸机使用时间,ICU住院时间、VAP发生率、住院费用。结果各项呼吸指标均有改善,差异具有统计学意义(P<0.05);试验组患者干预后呼吸机使用时间、住院时间较对照组短,住院费用较对照组低(P<0.05);干预后,试验组VAP发生8例,发生率为5.3%,对照组VAP发生36例,发生率为24.0%,2组比较差异具有统计学意义(P<0.05)。结论早期活动能够改善心外ICU患者的呼吸功能,缩短机械通气及住院时间,相应减少住院费用,降低VAP发生率,提高患者预后,值得临床推广。

Objective To explore the preventive effect of early activities on ventilator-associated pneumonia in patients with cardiovascular surgery.Methods 300 patients undergoing adult cardiac surgery were selected and randomly divided into experimental group and control group,with 150 cases in each group.The patients in control group were cared by the conventional nursing methods and the patients in experimental group were cared by early activity intervention on the basis of the conventional nursing methods.The various respiratory indicators,ventilator use time,ICU hospitalization time,VAP incidence,and hospitalization expenses were observed.Results After the intervention,the respiratory indicators were all improved,and the difference was statistically significant(P<0.05).The ventilator use time and hospitalization time of the patients in the experimental group were shorter than those in the control group,and the hospitalization expenses were lower than those in the control group(P<0.05).After the intervention,8 cases of VAP occurred in the experimental group,the incidence rate was 5.3%,and 36 cases of VAP occurred in the control group,the incidence rate was 24.0%,and the difference was statistically significant(P<0.05).Conclusion Early activity can improve the respiratory function of patients in cardiovascular surgery intensive care unit,shorten mechanical ventilation and hospitalization time,correspondingly reduce hospitalization expenses,reduce the incidence of VAP and improve the prognosis of patients.It is worthy of clinical promotion.
