摘要
目的探讨血清25-羟维生素D[25-(OH)D]、CD4^(+)、CD8^(+)水平与肺炎支原体(MP)感染后喘息的相关性。方法选取确诊的MP感染患者120例,感染后发生喘息者42例作为观察组,未发生喘息78例作为对照组。记录2组患者一般情况并测定C反应蛋白(CRP)、白细胞计数(WBC)等,分析患者MP感染后发生喘息的危险因素并比较25-(OH)D、CD4^(+)、CD3^(+)、CD8^(+)水平。Logistic回归分析影响肺炎支原体感染后喘息的危险因素,ROC分析25-(OH)D、CD4^(+)、CD3^(+)、CD8^(+)水平及三者联合预测MP感染后发生喘息的价值。结果MP感染后发生喘息主要与病程、咳嗽、肺部啰音、IgM抗体滴度有关;病程>7 d和IgM抗体滴度1∶160/1∶320是感染后发生喘息的重要独立危险因素(P<0.05)。ROC曲线分析显示,25-(OH)D、CD4^(+)、CD3^(+)、CD8^(+)水平联合预测MP感染后喘息的AUC为0.875,明显高于各项单独检测结果。结论25-(OH)D、CD4^(+)、CD3^(+)、CD8^(+)的水平与MP感染后发生喘息密切相关,可以作为预测喘息发生的指标。
Objective To explore the correlation between serum 25-hydroxyvitamin D(25-(OH)D),CD4^(+),CD8^(+)levels and wheezing after mycoplasma pneumoniae(MP)infection.Methods A total of 120 patients who were confirmed with MP were enrolled.Out of 120 patients,there were 42 cases with wheezing after infection in observation group and 78 cases without wheezing in control group.The general conditions,C-reactive protein(CRP),white blood cell count(WBC)and others were recorded.The risk factors of wheezing after MP infection in both groups were analyzed.The levels of 25-(OH)D,CD4^(+),CD3^(+)and CD8^(+)were compared between the two groups.The risk factors of wheezing were analyzed by Logistic regression analysis.The predictive value of 25-(OH)D,CD4^(+),CD3^(+),CD8^(+)and their combination for wheezing after MP infection was analyzed by ROC curves.Results Wheezing after MP infection was mainly related to disease course,cough,lung rales and IgM antibody titer.The disease duration more than 7d and IgM antibody titer of 1:160/1:320 were important independent risk factors of wheezing after infection(P<0.05).ROC curve analysis showed that AUC of 25-(OH)D combined with CD4^(+),CD3^(+)and CD8^(+)for predicting wheezing after MP infection was 0.875,significantly greater than that of single index.Conclusion The levels of 25-(OH)D,CD4^(+),CD3^(+)and CD8^(+)are closely related to the occurrence of wheezing after MP infection,which can be applied to predict wheezing.
