摘要
目的探讨微型种植体支抗治疗错[牙合]畸形患者的效果及对其龈沟液中细胞间黏附分子1(Intercellular adhesion molecule 1,CAM-1)及基质金属蛋白酶-2(Matrixmetalloproteinases 2,MMP-2)水平的影响。方法选取2016年2月至2019年3月于宜宾市第二医院治疗的错[牙合]畸形患者64例,根据随机数字表法,分为试验组与对照组,每组32例。对照组给予传统直丝弓正畸治疗,试验组给予微型种植体支抗治疗。观察并计算治疗前后中切牙轴倾角、磨牙位移、上中切牙转矩差;检测两组患者治疗后咀嚼效率和龈沟液中CAM-1、MMP-2、NO、IL-1β的含量,观察头影测量结果和不良反应。结果治疗后两组患者前后切牙轴倾角、磨牙位移、上中切牙转矩差、咀嚼效率差异有统计学意义(P<0.05);两组患者治疗前ANB、SNA、SNB、Angle、U1/L1、OJ、OB、L1/MP、U1/SN、CAM-1、MMP-2、NO、IL-1β差异无统计学意义(P>0.05),治疗后上诉指标均减小(P<0.05);试验组总不良反应发生率12.50%,对照组总不良反应发生率18.75%(P=0.491)。结论微型支抗体治疗口腔畸形的疗效好,对牙周组织的损伤小,有助于改善面部形态,且不良反应小,安全性高,适合应用于临床。
Objective The aim of this study was to investigate the effect of mini-implant anchorage on the treatment of malocclusion and the levels of intercellular adhesion molecule-1(CAM-1)and matrix metalloproteinases 2(MMP-2)in gingival crevicular fluid.Methods 64 patients with malocclusion who were treated in Yibin second hospital from February 2016 to March 2019 were divided into experimental group and control group according to random number table method,32 cases in each group.The control group received orthodontic treatment with traditional straight wire arch and the experimental group received micro-implant anchorage treatment.The difference of Torque difference of upper central incisor,molar displacement and axis inclination of central incisor were observed and calculated.The masticatory efficiency and the contents of CAM-1,MMP-2,NO and IL-1beta in gingival crevicular fluid of the two groups were measured after treatment.The cephalometric results and adverse reactions were observed.Results After treatment,there were significant differences in Torque difference of upper central incisor,molar displacement,axis inclination of central incisor and masticatory efficiency between the two groups(P<0.05).There was no significant difference in ANB,SNA,SNB,angle,U1/L1,OJ,OB,L1/MP,U1/Sn CAM-1,MMP-2,NO,IL-1β,between the two groups before treatment(P>0.05),and the indexes of appeal decreased after treatment(P<0.05).The total adverse reactions in the test group were 12.50%,and 18.75%in the control group(P=0.491).Conclusion The effect of mini-implant anchorage in the treatment of malocclusion is good,the damage to periodontal tissue is small,it is helpful to improve the facial morphology,and the adverse side effect is reduced and the safety is high,which is suitable for clinical application.
