应用前段光学相干断层扫描作为角膜缘干细胞缺乏的诊断参数测量干眼患者中央角膜上皮厚度的临床研究

Corneal Epithelial Thickness Of Dry Eye Measured Using Anterior Segment Optical Coherence Tomography as a Diagnostic Parameter for Limbal Stem Cell Deficiency

目的:利用前段光学相干断层扫描(AS-OCT),研究干眼(DE)患者中央角膜上皮厚度(CET),作为角膜缘干细胞缺乏的诊断参数,辅助诊断黏蛋白缺乏型DE。方法:选择2020年4月~2020年10月在佛山市中医院眼科门诊确诊的21例(42眼)DE患者作为观察组,另选取20例(40眼)正常人作为对照组。在观察组和对照组的AS-OCT图像上测量中央角膜上皮厚度(CET),CET通过OCT-CET1、CET2和CET3获得。结果:观察组的中央角膜上皮厚度CET1(41.57±5.34)μm、CET2(43.19±3.56)μm和CET3(42.14±5.27)μm,均低于临界值46.6μm,差异有统计学意义(P<0.05);对照组的中央角膜上皮厚度CET1(51.70±5.47)μm、CET2(51.53±3.97)μm和CET3(50.35±5.57)μm,均高于临界值46.6μm,差异有统计学意义(P<0.05);观察组的CET1、CET2、CET3均低于对照组,差异有统计学意义(P<0.05)。结论:观察组的CET低于对照组,AS-OCT CET可作为角膜缘干细胞缺乏的诊断参数辅助诊断黏蛋白缺乏型DE。

Objective Using anterior segment optical coherence tomography(AS-OCT),we investigated the central corneal epithelial thickness(CET)of the dry eye(DE)in patients with limbal stem cell deficiency as a diagnostic parameter to assists in the diagnosis of mucin deficient type DE.Method 21 patients(42 eyes)with DE diagnosed in the ophthalmic outpatient department of Foshan Hospital of Traditional Chinese Medicine from April 2020 to October 2020 were selected as the observation group,and another 20 healthy patients(40 eyes)were selected as the control group.Central corneal epithelial thickness(CET)was measured on AS-OCT images of the observation group and the control group.CET was obtained through OCT-CET1,CET2 and CET3.Results The thickness of central corneal epithelium CET1(41.57±5.34)μm、CET2(43.19±3.56)μm and CET3(42.14±5.27)μm in the observation group was all lower than the critical value of 46.6μm(P<0.05);The thickness of central corneal epithelium CET1(51.70±5.47)μm、CET2(51.53±3.97)μm and CET3(50.35±5.57)μm in the control group was all higher than the critical value of 46.6μm(P<0.05);CET1,CET2 and CET3 in the observation group were all lower than those in the control group(P<0.05).Conclusion CET in the observation group was lower than that in the control group,and AS-OCT CET could be used AS a diagnostic parameter for limbal stem cell deficiency to assist the diagnosis of mucin deficient type DE.
