摘要 目的观察重组人脑利钠肽对心力衰竭患者心功能、炎症指标、血浆N末端脑钠肽原、尿量的影响。方法80例心力衰竭患者,采用随机数字表法分为对照组及观察组,每组40例。对照组采用常规药物治疗,观察组在对照组的基础上联合重组人脑利钠肽治疗。比较两组患者的心功能、炎症指标、血浆N末端脑钠肽原、尿量水平。结果观察组患者的左室收缩末期内径(42.6±4.1)mm小于对照组的(45.3±4.5)mm,左室射血分数(48.5±5.8)%高于对照组的(44.5±5.1)%,差异具有统计学意义(P<0.05);两组患者的左室舒张末期内径、心脏指数、每搏输出量比较,差异无统计学意义(P>0.05)。观察组患者的白介素-6(36.4±4.6)ng/L、超敏C反应蛋白(1.3±0.4)mg/L、血浆N末端脑钠肽原(1132.5±484.0)pg/ml低于对照组的(51.2±5.2)ng/L、(2.7±0.5)mg/L、(1425.6±495.2)pg/ml,24 h尿量(1267.5±120.5)ml多于对照组的(1024.5±112.5)ml,差异具有统计学意义(P<0.05)。结论重组人脑利钠肽治疗心力衰竭的效果明显,可显著改善心功能和炎症指标水平。 Objective To observe the effect of recombinant human brain natriuretic peptide on cardiac function,inflammatory indexes,plasma N-terminal brain natriuretic peptide,and urine output in patients with heart failure.Methods A total of 80 patients with heart failure were divided into control group and observation group according to random numerical table,with 40 cases in each group.The control group was treated with conventional drugs,and the observation group was treated with recombinant human brain natriuretic peptide on the basis of the control group.The cardiac function,inflammatory indexes,plasma N-terminal pro-brain natriuretic peptide and urine output of the two groups were compared.Results The left ventricular end-systolic diameter(42.6±4.1)mm of the observation group was smaller than(45.3±4.5)mm of the control group,and the left ventricular ejection fraction(48.5±5.8)%was higher than(44.5±5.1)%of the control group.The difference was statistically significant(P<0.05).There was no statistically significant difference in the left ventricular end-diastolic diameter,cardiac index,and stroke volume between the two groups(P>0.05).The interleukin-6(36.4±4.6)ng/L,high-sensitivity C-reactive protein(1.3±0.4)mg/L,and plasma N-terminal pro-brain natriuretic peptide(1132.5±484.0)pg/ml of the observation group were lower than(51.2±5.2)ng/L,(2.7±0.5)mg/L,(1425.6±495.2)pg/ml of the control group,and the 24 h urine output(1267.5±120.5)ml was more than(1024.5±112.5)ml of the control group.All differences were statistically significant(P<0.05).Conclusion Recombinant human brain natriuretic peptide is effective in the treatment of heart failure and can improve cardiac function and inflammatory indexes.
