2020年自贡市一起肺炎克雷伯菌感染引起的流感样暴发疫情调查

Investigation on an influenza-like outbreak infected with Klebsiella pneumoniae in Zigong city, 2020

目的调查分析2020年7月四川省自贡市X公司发生流感样聚集性疫情的原因,为类似疫情鉴别处置提供参考依据。方法在X公司员工中搜索病例,对疑似病例开展问卷调查,采集病例咽拭子开展新型冠状病毒肺炎(COVID-19)病毒核酸检测和微流体呼吸道多病原筛查。结果共发现病例44例,罹患率6.3%(44/702),主要症状为咽痛(34例、占77.3%)、头晕/头痛(25例、占56.8%)及发热(21例、占47.7%)。8例发热病人咽拭子中检测出肺炎克雷伯菌7例(占87.5%)、流感嗜血杆菌6例(占62.5%)。COVID-19病毒核酸检测均为阴性。结论本次聚焦性疫情为肺炎克雷伯菌感染引起的普通感冒暴发事件。在发生疑似COVID-19聚集性疫情时,除进行COVID-19病毒核酸检测外,可结合流行病学调查、临床症状和呼吸道多病原检测技术实现疾病的快速判定。

Objective To investigate and analyze the cause of an influenza-like clustering epidemic in X company in Zigong city of Sichuan province in July 2020, and provide reference for identification and disposal of similar epidemics. Methods The cases were searched among the employees of X company and the questionnaire were carried out in suspected cases, and the throat swabs were collected for COVID-19 nucleic acid testing and microfluidic respiratory multi-pathogen screening.Results Totally 44 cases were found out, with attack rate of 6.27%(44/702). Sore throat(34 cases, occupied 77.3%), dizzy/headache(25 cases, occupied 56.8%) and fever(21 cases, occupied 47.73%) were the main symptoms. The Klebsiella pneumoniae and haemophilus influenzae were detected in the throat swabs of 7 cases(87.5%) and 6 cases(62.5%),respectively among the 8 cases with fever. The results of COVID-19 testing were negative. Conclusions This clustering epidemic is a common cold outbreak infected with klebsiella pneumoniae. In the event of a suspected COVID-19 clustering epidemic, in addition to the detection of novel cronavirus, the rapid identification of the disease can be realized by combining epidemiological survey, clinical symptoms and respiratory multi-pathogen detection technology.
