胰腺癌术后发生肝转移的影响因素分析

To Investigate the Influencing Factors of Liver Metastasis after Pancreatic Cancer

目的分析胰腺癌术后发生肝转移的影响因素。方法选取胰腺癌术后肝转移患者41例作为肝转移组,以1∶1配比选取同期胰腺癌术后无肝转移患者41例作为无肝转移组。收集2组性别、年龄、体质量指数(BMI)、术前肝功能分级、癌组织分化程度等资料,统计分析影响因素。结果BMI、术前肝功能分级、癌组织分化程度、有无脉管癌栓、是否合并脂肪肝、有无术后化疗是胰腺癌术后发生肝转移的影响因素(P<0.05);logistic回归分析显示,BMI>23 kg/m^(2)、术前肝功能分级B级、癌组织低分化、脉管癌栓是胰腺癌术后肝转移的危险因素,合并脂肪肝、术后化疗是胰腺癌术后肝转移的保护因素(P<0.05)。结论BMI>23 kg/m^(2)、术前肝功能分级B级、癌组织低分化、脉管癌栓是胰腺癌术后肝转移的危险因素,合并脂肪肝、术后化疗是胰腺癌术后肝转移的保护因素,临床可据此制定防控方案,降低胰腺癌患者术后肝转移风险,提高远期效果。

Objective To analyze the influencing factors of liver metastasis after pancreatic cancer.Methods 41 patients with liver metastasis after pancreatic cancer operation were selected as liver metastasis group,41 patients with no liver metastasis after pancreatic cancer were selected as no liver metastasis group.Gender,age,body mass index(BMI),preoperative classification of liver function and differentiation of cancer tissues were collected,Statistical analysis of influencing factors.Results BMI,preoperative classification of liver function,differentiation of cancer tissues,presence or absence of vascular tumor thrombus,fatty liver and chemotherapy were the influencing factors of postoperative liver metastasis(P<0.05);logistic regression analysis showed,BMI>23 kg/m^(2),preoperative liver function grade B,low differentiation of cancer tissue,vascular tumor thrombus were risk factors for postoperative liver metastasis in pancreatic cancer,Combined fatty liver and postoperative chemotherapy were protective factors for postoperative liver metastasis of pancreatic cancer(P<0.05).Conclusion BMI>23 kg/m^(2)、preoperative liver function grade B,low differentiation of cancer tissue and vascular tumor thrombus are risk factors for postoperative liver metastasis of pancreatic cancer,The combination of fatty liver and postoperative chemotherapy is a protective factor for postoperative liver metastasis of pancreatic cancer,Clinical can be based on the formulation of prevention and control programs,Reducing the risk of postoperative liver metastasis in patients with pancreatic cancer,Improve the long-term effect.
