上皮性卵巢癌术后复发的相关影响因素分析

Analysis of Related Factors of Postoperative Recurrence of Epithelial Ovarian Cancer

目的分析上皮性卵巢癌(EOC)术后复发的相关影响因素。方法选择124例EOC患者作为研究对象,入选患者均行手术及化疗治疗,并进行长期随访。根据5年随访结果将患者分为复发组(51例)和未复发组(73例)。采用单因素和多因素Logistic回归分析EOC术后复发的相关影响因素。结果两组年龄、分娩次数、组组织学类型、有无腹腔积液、手术方式、化疗方案比较,差异无统计学意义(P>0.05);复发组中肿瘤分期Ⅲ+Ⅳ、术后残留肿瘤>1 cm、术前血清糖类抗原125(CA125)≥200 U/ml、术前血清白蛋白(ALb)≤35 g/L、术前血清附睾蛋白4(HE4)≥200 pmol/l占比均高于未复发组,差异有统计学意义(P<0.05);FIGO肿瘤分期Ⅲ+Ⅳ、术后残留肿瘤>1 cm、术前CA125≥200 U/ml、术前ALb>35 g/l、术前HE4≥200 pmol/l均是EOC患者术后复发独立危险因素。结论EFIGO肿瘤分期Ⅲ+Ⅳ、术后残留肿瘤>1 cm、术前CA125≥200 U/ml、术前ALb>35 g/l、术前HE4≥200 pmol/l均是EOC患者术后复发独立危险因素,临床应采取针对性措施,延长患者生存时间。

Objective To analyze the related factors of postoperative recurrence of epithelial ovarian cancer(EOC).Methods 124 patients with EOC were selected and treated with surgery and chemotherapy,and were followed up for a long time.According to the results of 5-year follow-up,the patients were divided into recurrence group(51 cases)and non recurrence group.Results There was no significant difference between the 2 groups in age,number of deliveries,histological type,presence or absence of peritoneal effusion,mode of operation and chemotherapy regimen(P>0.05)(CA125)≥200 U/ml,preoperative serum albumin(ALB)≤35 g/l and preoperative serum epididymal protein 4(HE4)≥200 pmol/l were significantly higher than those in the non recurrence group(P<0.05);FIGO tumor stageⅢ+Ⅳ,postoperative residual tumor>1 cm,preoperative CA125≥200 U/ml,preoperative ALB>35 g/l and preoperative HE4≥200 pmol/l were independent risk factors for postoperative recurrence in EOC patients.Conclusion EFIGO tumor stageⅢ+Ⅳ,postoperative residual tumor>1 cm,preoperative CA125≥200 U/ml,preoperative ALB>35 g/l and preoperative HE4≥200 pmol/l are independent risk factors for postoperative recurrence in EOC patients,clinical targeted measures should be taken to prolong the survival time of patients.
