脊柱骨肿瘤患者术后1年内复发的相关影响因素

Analysis of Factors Influencing Recurrence in Patients with Spinal Bone Tumor Within 1 Year After Operation

目的分析脊柱骨肿瘤患者术后1年复发的相关影响因素。方法选取脊柱骨肿瘤患者71例,统计1年复发情况,收集复发和未复发患者年龄、性别、肿瘤直径、胰岛素样生长因子ⅡmRNA结合蛋白3(IMP3)表达情况、Tomita分型、双磷酸盐使用情况等信息,分析影响因素。结果本组71例患者,术后1年复发27例,未复发44例,复发率38.03%(27/71)。IMP3表达情况、Tomita分型、是否使用双磷酸盐、是否术后放疗、侵袭范围、累及节段数量、手术方式是脊柱骨肿瘤患者术后1年复发的影响因素(P<0.05);logistic回归分析显示,IMP3表达阳性、Tomita分型Ⅴ~Ⅵ、侵袭至间室外、累及多节段、次全椎节切除术为脊柱骨肿瘤患者术后1年复发的危险因素,使用双磷酸盐、术后放疗为脊柱骨肿瘤患者术后1年复发的保护因素(P<0.05)。结论IMP3表达阳性、Tomita高分型、侵袭至间室外、累及多节段、次全椎节切除术为脊柱骨肿瘤患者术后1年复发的危险因素,使用双磷酸盐、术后放疗为脊柱骨肿瘤患者术后1年复发的保护因素,临床应据此加强临床干预防控力度,以降低患者术后复发风险。

Objective To analyze the influencing factors of 1-year postoperative recurrence in patients with spinal bone tumor.Methods 71 patients with spinal tumor were selected,Statistics of 1 year recurrence were analyzed,to collect information on age,sex,tumor diameter,insulin-like growth factor ii mRNA binding protein 3(IMP3)expression,Tomita typing,and bisphosphate use in recurrent and non-recurrence,and analyze the influencing factors.Results Of the 71 patients,27 cases had recurrence in 1 year,44 cases had no recurrence,The recurrence rate was 38.03%(27/71).IMP3 expression,Tomita type,whether to use bisphosphate,whether postoperative radiotherapy,invasion range,the number of involved segments,the operation mode was the influence factor of 1 year postoperative recurrence in patients with spinal bone tumor(P<0.05);logistic regression analysis showed that IMP3 expression positive,Tomita typingⅤ~Ⅵ,invasive range outdoor,involving multiple segments,subtotal vertebral resection were risk factors for 1-year postoperative recurrence in patients with spinal bone tumors.Bisphosphate,postoperative radiotherapy were protective factors for 1-year postoperative recurrence in patients with spinal bone tumors(P<0.05).Conclusion IMP3 Table-positive,Tomita high typing,invasive outdoor,multi-segment and subtotal vertebral resection are the risk factors for 1-year postoperative recurrence of spinal bone tumors,Using bisphosphate and postoperative radiotherapy as protective factors for 1-year postoperative recurrence in patients with spinal bone tumors,Clinical intervention and prevention should be strengthened accordingly to reduce the risk of postoperative recurrence.
